Short Description
This is a one-year noncompetitive grant for $100,000.00 awarded to the East Side Health Department. Sexually transmitted infections (STIs) remain a significant public health problem in Illinois. In 2023, Illinois ranked 31st for reported primary and secondary syphilis, 11th for chlamydia, and 13th for gonorrhea in the United States. East Side Health District, East St. Louis, IL, is the lead agency in southern Illinois that provides comprehensive sexual health services to the uninsured and underinsured population. In 2023, 51% of all reported gonorrhea cases in Illinois consisted of African Americans. The Southern Illinois Region, which is more rural and has higher disparities and the highest reported African American to other race STI disparity ratio in Illinois (e.g., African American: White, African American: Hispanic), historically lacks easily accessible medical care that provides comprehensive sexual health services. The East Side Health District area consists of the poorest communities and a lack of health care services for sexual health, and the majority, if not all, of the clientele are African American and/or the uninsured/underinsured population. East Side Health District ranks in the top counties in Illinois for the number of gonorrhea cases reported and is the leading county in the number of morbidity cases in southern Illinois. Clients cross state lines between the East St Louis area and the St. Louis area in Missouri. The disease intervention specialists (DIS) at ESHD counsel and educate all gonorrhea cases positive for sexually transmitted infections (STIs) on STIs and HIV, as well as their health care providers and the community at large. The services provided by this grant opportunity are essential to help prevent further transmission of STIs and the resulting costly medical complications such as pelvic inflammatory disease, ectopic pregnancy, infertility, and neonatal death and disability. The Illinois Department of Public Health shall fund East Side Health District to support STI prevention and containment activities in the East Side Health District (ESHD) jurisdiction within St Clair County, with a focus on gonorrhea prevention for one year (12 months) starting January 1, 2026, through December 31, 2026. This grant opportunity shall help the Illinois Department of Public Health STI Section (ISS) accomplish the Centers for Disease Control and Prevention’s (CDC) Strengthening Sexually Transmitted Diseases Prevention and Control for Health Departments (STD PCHD) federal grant objectives and activities.

Objective
Agency shall deliver the following in the East Side Health District jurisdiction; 1. Provide STI prevention services predominantly among adolescents/young adults, and pregnant women who are people of color, specifically the African American community. 2. Provide STI safety net services for any uninsured and underinsured residents. 3. Provide disease intervention specialist (DIS) counseling, partner services, risk assessment, sex partner referral, treatment and culturally competent STI prevention and medical services for all clients seen at ESHD STI clinic. 4. Provide DIS counseling, partner services, risk assessment, and sex partner referral for at least 80% of ESHD residents reported with an STI and was seen by a healthcare provider located in ESHD jurisdiction as infected with gonorrhea. 5. Conduct disease investigation and intervention for all untreated cases of chlamydia, gonorrhea, and syphilis reported by a healthcare provider located in ESHD jurisdiction. 6. Provide technical assistance to heath care providers and community organizations including but not limited to; initiating, reviewing and transmitting mandatory STI case reports to ESHD and providing community engagement, outreach, and/or educational activities. 7. Promote STI prevention and treatment services according to CDC guidelines. 8. Monitor and report cases of chlamydia, gonorrhea, and syphilis as required by law. 9. Provide an Outbreak Response Plan to ISS by December 31, 2019. a. Plan shall be reviewed by ESHD staff at least annually. b. If there are new or updated content in the ESHD’s Outbreak Response Plan after initial submission during this grant period, an updated plan shall be submitted to ISS. 10. Respond to chlamydia, gonorrhea, and syphilis incidence and prevalence trends and outbreaks as reported by ISS. 11. Develop and utilize targeted approaches that result in programmatic improvement, within their agency, that impact communities with high morbidity.
