The Illinois Department of Public Health Office of Health Protection is accepting applications from government entities that operate licensed beaches on Lake Michigan for the purpose of ensuring basic levels of protection are maintained at the community level for recreational water quality monitoring and public notification. The Illinois Department of Public Health (IDPH) recognizes that partnerships are necessary to maximize resources, to reduce duplication of effort, and to improve quality, and efficiency of health care services. Fecal contamination of our nation's recreation waters originates from many sources. People who swim and recreate in water contaminated with fecal pollution are at an increased risk of becoming ill because of pathogens from the fecal matter. The goal of the Environmental Health Beach Program is to provide assistance to units of local government monitoring beaches operated on Lake Michigan. The Grantee will provide the following services and agrees to act in compliance with all State and federal statutes and administrative rules.

34.1 The Department is authorized to make this grant pursuant to the BEACHES ENVIRONMENTAL ASSESSMENT AND COASTAL HEALTH ACT OF 2000 (Beach Act)(33 U.S.C. §§ 1251 et seq.). The Beach Act amended the Clean Water Act to add section 406, which authorizes the EPA to award grants to states or local governments to develop and implement beach monitoring and assessment programs.

Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? The name and address of the appealing party ? Identification of the grant ? A statement of reasons for the appeal ? If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. o The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. o The appealing party must supply any additional information requested by the agency within the time period set in the request. o The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required. Documentation of the appeal determination shall be sent to the appealing party and must include the following: o Standard description of the appeal review process and criteria o Review of the appeal o Appeal determination o Rationale for the determination In addition to providing the written determination, the grant-making office may do the following: o Document improvements to the evaluation process given the findings and re-review all submitted applications. o Document improvements to the evaluation process given the findings and implement improvements into the following year's grant evaluation process. o Provide written notice to the appealing party as to how the identified actions will be remedied. For support of the ordinary and contingent expenses of the following safety net hospitals to preserve their sustainability in accordance with the Safety-Net Hospital Health Equity and Access Leadership Grant Program authorized under Section 2310- 715 of Department of Public Health Powers and Duties Law, as enacted in Public Act 102-886, and that participate in the State’s Medicaid managed care program.

Grantees are subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90. As required by the terms of the grant agreement.

Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken. Within the USEPA’s National Beach Guidance and Required Performance Criteria of Grants document (NBG), Beach Notification Thresholds are provided for Beach Act States. Beach jurisdictions have the option to use those Beach Action Values (BAV) provided in the NBG. The majority of the beach jurisdictions along Lake Michigan use a BAV of 235 colony forming units (cfu) per 100 ml of E. coli as the threshold for issuing notifications to the public. This is based on the 1986 USEPA document, Ambient Water Quality Criteria for Bacteria and is also referenced in the NBG.N/A
