            PROGRAM INFORMATION
            
                Short Description
                The Illinois Department of Public Health Center for Minority Health Services is requesting applications from community-based organizations that serve minority populations to provide education and outreach services for the specific purpose of increasing minority enrollment and reenrollment in ADAP. Recipients must design MAI funded services that meet the specific intent and parameters of the funding.  Programs should be unique, culturally innovative to the specified target group, and increase ongoing efforts of the Department to support education and outreach services to increase the number of eligible racial and ethnic minorities who have access to treatment through ADAP. Successful applicants will show the ability to provide supplemental support education and outreach services to increase the number of eligible racial and ethnic minorities who have access to treatment through ADAP. This includes working with individuals who have been lost to care and working with the list of people that have been identified as lost to care that is provided by DPH.   Successful applicants must demonstrate their capacity to successfully provide services to their selected population, and demonstrate experience working with the target population. Given the legislative intent of the RWHAP Part B MAI funding to increase minority enrollment in ADAP, the recipient must be able to trace the RWHAP Part B MAI activity to the client’s enrollment into ADAP.

                Legally recognized organizational entities are eligible to apply for this funding.  Applications from individual persons will not be considered.  Grants may be made to individual organizations or to lead agencies distributing sub-grants to partnering individual organizations.  Only organizations based in Illinois are eligible for funding
             
            
                Beneficiary Eligibility
                HIV positive racial and ethnic minorities who need access to treatment through ADAP. This includes working with individuals who have been lost to care and working with the list of people that have been identified as lost to care that is provided by DPH

                Award Procedures
                Applicants will receive notification via Egrams of the award once all of the applications in E-grams are reviewed and scored.

                Appeals
                For merit based reviews, only the evaluation process is subject to appeal. Competitive grant appeals are limited to the evaluation process. Evaluation scores or award decisions may not be protested.
             
            
                Renewals
                This grant is a three -year grant competitively awarded in year 1 with the option of two non-competitive renewal. Funding for subsequent years shall be contingent upon available funds, and applicant performance evaluation conducted by the Illinois Department of Public Health.
             
            
            
                Uses and Restrictions
                Funds can be used for all reasonable, allocated and allowable costs. There is a 10% cap on indirect costs.

                All reporting will be conducted through Provide, monthly reimbursements and quarterly work plan reports
             
            
                Audits
                Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget.  See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
             
            
                Records
                Records Retention. Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).  If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
