            PROGRAM INFORMATION
            
                Short Description
                The purpose of the Illinois Hospital Preparedness Pediatric Program is to prevent and reduce childhood and adolescent death and disability resulting from illness and injury by enhancing emergency preparedness this work is accomplished in collaboration with the Emergency Medical Services for Children (EMSC). The EMSC program will provide guidance and assistance to hospitals, regional health care coalitions and other healthcare related entities aimed at improving pediatric emergency preparedness.

                This funding is made available via Section 319-C of the Public Health Service (PHS) Act (title 42 United States Code (USC) 247d-3b), as amended, and Section 311 of the PHS Act (title 42 USC 243), subject to available funding and other requirements and limitations

                Objective
                The Hospital Preparedness Program Pediatric Grant supports pediatric education/training programs and provisions of resources that aim to enhance pediatric emergency preparedness in collaboration with the 10 Illinois Healthcare coalitions which include a diverse membership including: Hospitals, Emergency Medical Service, Local Health Departments and Emergency Management Agencies.
Payments to the Grantee are subject to the Grantee’s submission and certification of actual and necessary eligible costs in conformity with approved Budget and any documentation as required by the Department. Payment shall be initiated upon the Department’s approval of actual and necessary eligible costs and cash amount requested for reimbursement of those costs.

                Applicant Eligibility
                Only Illinois Licensed hospitals designated as Regional Hospital Coordinating Centers (RHCCs) may apply for this grant. An entity may not apply for a grant until the entity has registered and pre-qualified through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, Grantee Links tab. Registration and pre-qualification are required annually. During pre-qualification, verifications are performed including a check of Federal Debarred and Suspended and status on the Illinois Stop Payment List. An automated email notification to the entity alerts them of “qualified” status or informs how to remediate a negative verification (e.g. inactive Dun and Bradstreet, not in good standing with the Secretary of State). A federal Debarred and Suspended status cannot be remediated

                Preapplication Coordination
                Technical Assistance Webinar. All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State.                 Pre-applications and letters of intent are not required. Applications must be fully and properly completed and submitted by the deadline.                 Criteria Selecting Proposals
                This is a formula grant program. The applicants must submit their application in accordance with fiscal year Funding Opportunity Announcement. The applications are reviewed and approved applications are processed immediately. Applications approved with conditions, require the applicant to resubmit supporting information within a prescribed time frame. Upon review and approval, the revised application can then be submitted to Grants Management for processing and issuing a notice of grant award.
             
            
                Award Procedures
                The application will be reviewed by IDPH OPR at which time all questions in the above section will be taken into consideration. Applications that are not correctly completed or need clarification will be returned to the applicant for correction and resubmission. IDPH comments and clarification requests can be accessed through the icons found on the “Index” tab screen in the “Comments” Column - and by the “Approver Comments” at the bottom. IDPH may also deny an applicant a grant award if they are or become ineligible. If any reviewer states they have a potential or appearance of a conflict of interest, or an applicant state there is a conflict of interest, or the appearance thereof, with any of the reviewers or staff listed under “G. State Awarding Agency Contact(s)”, the application will be assigned to another reviewer. Once approved by IDPH, the applicant will be notified electronically via email with instructions on how to print, sign and submit their grant to IDPH for final execution.
Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

                Appeals
                Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b.  Appeals must be received within 14 calendar days after the date that the grant award notice was published.  Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information.   The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request.    The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
             
            
            
            
                Uses and Restrictions
                Allowable - All grant funds must be used for items that are necessary and reasonable for the proper and efficient performance of the grant and may only be used for the purposes stated in your grant agreement, work plan, and budget. Items must not be restricted by, and must comply with, all applicable state and federal regulations. Allocable- Grant-funded costs must be chargeable or assignable to the grant in accordance with relative benefits received. The allocation methodology should be documented and should be consistent across funding sources for similar costs. Illinois Department of Public Health - Office of Performance Management Page 6 of 9 appropriately allocated (pro-rated) between this grant and other funding sources as applicable. This is especially important to describe and document if the item appears to have significant non-grant uses such as a purchase that directly supports many or all hospital programs and is not strictly considered an emergency preparedness capability resource.                 Audits
                Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.

                Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).

                Program Accomplishments
                The Hospital Preparedness–Pediatric Preparedness grantee successfully served as co-chair of the State Pediatric Preparedness Workgroup, actively participating in bi-monthly workgroup meetings. The grantee coordinated meeting agendas and minutes, collaborated on document reviews with the co-chair, and ensured all completed products adhered to national guidelines and best practices. Additionally, they maintained detailed records, including meeting agendas, minutes, and sign-in rosters, for each session. Progress was consistently reported through quarterly updates to the Illinois HPP Program and shared with the Pediatric Preparedness Workgroup and the Illinois EMSC Advisory Board during their meetings.

                Example Projects
                The Hospital Preparedness- Pediatric Preparedness grantee implemented an online School Nurse Emergency Care (SNEC) course in conjunction with UIC Public Health Learning, and assess course completion rates, post-test scores, user feedback, and course evaluations.
