                Short Description
                This grant activity will provide infectious disease and stewardship expertise to outpatient healthcare facilities. Grantee will encourage outpatient facilities to implement the Core Elements of Outpatient Stewardship as outlined by the CDC and provide consultation and support for implementation of a policy or practice to improve antibiotic prescribing in participating facilities. Grantee activities will include tracking and evaluating antibiotic prescribing data to provide regular feedback to outpatient clinicians to improve prescribing practices. Grantee will develop/provide resources to participating facilities as relevant, including educational materials for patients. Grantee will work closely with the Illinois Department of Public Health (IDPH) Division of Patient Safety and Quality and CDC.

                Objective
                The purpose of this grant is to improve clinician knowledge and practices related to antimicrobial prescribing anduse, reduce inappropriate antimicrobial prescribing, and mitigate antimicrobial resistance to optimize clinicaloutcomes in outpatient healthcare settings in the state of Illinois.

                Applicant Eligibility
                Regardless of the source of funding (federal pass-through or State), all grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award.The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State.                 Merit-based review - the application will be reviewed by a panel and scored based on the criteria described in Section E of the NOFO. Expenses reimbursed based on monthly reporting.

                For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer.                 Renewals
                Subject to satisfactory performance and availability of funding, may be renewed for one additional year, for a total of 2 years maximum
             
            
            
                Uses and Restrictions
                Funding may not be used for research, clinical care, propaganda or lobbying, and in general not for furniture or equipment in accordance with federal guidelines.
             
            
                Reports
                Monthly performance and financial reports will be required.
