                Short Description
                The Bright Smiles from Birth II Oral Health Training Grant provides funds to outreach, train, educate and incorporate oral health intervention into the workflow of primary care providers that care for infants and young children. The goal is to improve oral health status through incorporation of oral health promotion, prevention, risk assessment, providing of anticipatory guidance and necessary tools that enable parent/caregiver action, including the application of fluoride varnish, and coordination to professional oral care all in efforts to improve the oral health status and limit impact of oral disease in infants and young children. Dental screening as part of the physical examination helps identify children who require further evaluation by a dentist and establish a dental home. To maintain program consistency and curriculum standards with content experts, the grant will be administered and housed by Illinois Chapter, American Academy of Pediatrics (ICCAP) as they have the capacity and prior work experience needed. Under the grant, ICAAP will 1) Maintain continuing education credit for relevant professional licenses attending the training sessions. 2) Build capacity of front-line, public health-minded pediatricians with preparatory education and training readying them to respond to common urgent general and oral health problems. 3) Continue expansion of statewide training and promotion of oral health. 4) Present project outcomes and boost training session through additional live web/based session. 5) Maintain and update the online learning management system training curriculum and resources on the program website.

                Objective
                The objective of this programmatic funding will be to decrease oral diseases, mainly dental caries in infants and children to age 6 by expanding the use of evidence-based prevention and oral assessment in the medical home and as part of well-child visits.
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws.

                Preapplication Coordination
                Regardless of the source of funding (federal pass-through or State), all grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, https://gata.illinois.gov/, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI (Unique Identity ID) number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered.                 Criteria Selecting Proposals
                Upon completion of the submission and review process, each successful grantee will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health. A Notice of State Award (NOSA) shall be issued to the finalists who have successfully completed all grant award requirements and have been selected to receive grant funding. The NOSA will specify the funding terms and specific conditions resulting from applicable pre-award risk assessments. The Illinois Department of Public Health (IDPH) is exempt from utilizing the standard NOSA issued on the GATA Grantee Portal. Successful applicants will receive an email notification from EGrAMS and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature in EGrAMS and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH.
             
            
                Award Procedures
                This grant is non-competitive. Application review and approval will be completed by the IDPH grant monitor based on prior grant guidelines, timely submission of reports and fiscal monitoring.

                Appeals
                Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b.  Appeals must be received within 14 calendar days after the date that the grant award notice was published.  Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information.   The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request.    The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
             
            
                Renewals
                There are no renewal awards for this grant program. Grantees must submit a new application for each grant cycle.
             
            
            
                Uses and Restrictions
                Indirect costs not allowed. Administrative costs are capped at 10% of your total budget. These are necessary and reasonable costs needed to administer the grant that are not related to the direct provision of services. Allowable costs include: • Accounting, budgeting, financial and cash management • Procurement and purchasing • Personnel and property management • Payroll, audit and general legal services • Oversight and monitoring of administrative activities • Developing information systems and procedures related to administrative function. All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted by deadline on the grant agreement. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the following general criteria: • Be necessary and reasonable for proper and efficient administration of the program and not be a general expense required to carry out the overall responsibilities of the grantee organization. • Be authorized or not prohibited under federal, state or local laws, or regulations. • Conform to any limitations or exclusions set forth in the applicable rules, program description, budget preparation instructions or grant agreement.
             
            
                Reports
                Monthly progress and financial reports shall be submitted to the Department. The Grantee shall keep adequate records relating to its administration of a project, particularly relating to all incurred costs. These records shall be available for audit by the Department and the State Auditor General. The Department reserves the right to conduct on-site visits with the purpose of reviewing or auditing a project. All records shall be retained in accordance with the State Records Act [5 ILCS 160]. Grantees are subject to the record retention requirements and auditing standards stipulated by GATU (see GATA Rules Section 7000.430(a) and Section 7000.90). Monthly Progress Reports and Reimbursement Certification Forms (RCF) shall be submitted electronically through EGrAMS within 30 calendar days following the end of each progress period.
             
            
                Audits
                Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
             
            
                Records
                Grantees are required to maintain grant accounting records for a minimum of 3 years after the end of a grant period. If any litigation, claim, negotiation, audit or other action involving the record has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later. More detailed information regarding retention requirements is provided in 45 CFR Parts 74 and 92.
