            PROGRAM INFORMATION
            
                Short Description
                The Healthcare Telementoring Expansion grant program will provide funding to an eligible organization to implement Phase II of an established workplan to establish or expand technology-enabled telementoring learning services. As determined by a competitive application process for Phase I (planning), The University of Chicago is the preferred applicant for continuation funding based on successful completion of a detailed plan for telementoring expansion and an appropriate budget proposal. The primary goal of the program is to enhance health care outcomes for patients with a variety of conditions and improve the overall health of Illinois residents. Per Illinois legislation, Health Care Tele mentoring means the following: 1. Based on interactive video or phone technology that connects groups of local health care providers or professionals in urban and rural underserved areas with specialists in regular real-time collaborative sessions; 2. That is designed around case-based learning and mentorship; and 3. Helps local health care providers or professionals gain the expertise required to more effectively provide needed services.

                Public Act 103-0588 Sec. 2310-730. Health care tele mentoring. (a) Subject to appropriation, the Department shall designate one or more health care tele mentoring entities based on an application to be developed by the Department. Applicants shall demonstrate a record of expertise and demonstrated success in providing health care tele mentoring services. The Department may adopt rules necessary for the implementation of this Section. Funding may be provided based on the number of health care providers or professionals who are assisted by each approved health care tele mentoring entity and the hours of assistance provided to each health care provider or professional in addition to other factors as determined by the Director.

                Objective
                The Healthcare Tele mentoring Expansion grant program has the primary objectives of enhancing outcomes for patients with a variety of health conditions and improving the overall health of Illinois residents. Tele mentoring programming or service expansion should include the following components: • Based on interactive video or phone technology that connects groups of local health care providers or professionals in urban and rural underserved areas with specialists in regular real-time collaborative sessions. • Designed around case-based learning and mentorship. • Help local health care providers or professionals gain the expertise required to more effectively provide needed services. Specific objectives have been identified by the University of Chicago in Phase 1 (planning), which are to be integrated into a workplan that can be measured against expected outcomes in a reasonable and timely manner. The grant application will include detailed objectives to be acknowledged, which include those related to planned activities, participant demographics, reporting outcomes, etc.
Funds must be used during the period of July 1, 2025, to June 30, 2026. Grant award is for one year only and may be renewable for one additional year based on availability of funds. The grant is planned to be awarded on a non-competitive basis due to eligibility criteria being met by The University of Chicago within Phase 1. Other program terms, including work plan and budget, are subject to review and approval by IDPH prior to execution of grant agreement and initiation of grant program.

                Criteria Selecting Proposals
                This grant award will be selected on a non-competitive basis with consideration for eligibility criteria and performance of prior grantee(s) from Phase 1 (planning) of the Healthcare Tele mentoring Expansion grant project. The University of Chicago is the only applicant to have met this eligibility criteria
             
            
            
                Deadlines
                The application must be completed by the date included on the associated non-competitive funding agreement. Early submission of the grant application is encouraged to potentially expedite the review and award process.
             
            
                Range of Approval or Disapproval Time
                Application review and award determination is variable based on potential revisions required to grant agreement and availability of individual or team reviewers. Expected duration of review is within 10 business days from application deadline.
             
            
                Appeals
                Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b.  Appeals must be received within 14 calendar days after the date that the grant award notice was published.  Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information.   The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request.    The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
             
            
                Renewals
                The grantee may be eligible for one additional year of funding through a non-competitive award process subject to availability of State funding and performance of the program compared to expected deliverables/outcomes.

                Requirements will be detailed in the non-competitive funding agreement. Monthly reporting on program progress and performance is expected.
             
            
                Audits
                Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
             
            
                Records
                Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
