Short Description
Issue of Permits: Local Health Department will conduct permitting and inspection of closed loop and water wells to assure that wells are constructed and located properly. Grantee also investigates complaints, ensures any violations are corrected, collects samples, provides education to public on proper construction, and be compensation for each water well and closed loop well permit issued.

Objective
Applicants are expected to: • Provide services between October 1, 2017 and September 30, 2018 • Complete submittal of the application by September 15, 2017. • Expected outcomes for the Potable Water Supply Program is to reduce contamination of groundwater and water systems through the proper construction of water wells, related water system components and closed loop wells. • Local Health Departments will conduct permitting and inspection of closed loop and private water wells to assure that wells are constructed and located properly.

This grant is only available to certified local health departments acting as agents for IDPH that perform the groundwater permit program. The grant provides reimbursement to Grantees that act as agents for IDPH. Applicants need to show acknowledgement that Grantee agrees to provide adequate staff to support all the initiatives and fulfill all of the requirements of issuing permits, inspecting installations and inspecting sealing of water and closed loop wells.

Preapplication Coordination
Funds awarded to successful applicants will be provided as fixed rate grant payments. Grantee routinely submits permit activity and payments to program staff. On a quarterly basis, IDPH calculates reimbursements equal to 75% of submitted payments from grantee. After program review and approval of reimbursements, a voucher will be prepared and processed through the State of Illinois Comptroller for payment.
Application Procedures
All applications must be submitted via the Illinois Department of Public Health’s Electronic Grants Administration and Management System (EGrAMS). All sections of the application must be complete
Criteria Selecting Proposals
This is a non-competitive grant. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any required information will be returned to the applicant for correction and re-submission. Because the LHPG is a non-competitive, formula-based grant program, all eligible applicants will receive a grant award.
Award Procedures
Upon final approval by IDPH of the EGrAMS grant application, the applicant will be sent a grant agreement through EGrAMS. Each applicant will be required to print the grant agreement, sign it and return it to IDPH for execution. After execution, a copy of the executed grant agreement will be provided to the applicant.

Formula Matching Requirements
Each certified local health department receives an estmated award amount based upon the number of permits submitted to IDPH the previous fiscal year.
Uses and Restrictions
The Department has had “Agreements for the Conducting of a Potable Water Supply Program” for more than 16 years. The grants are for disseminating funds received by the Department through local health departments, who act as agents of the Department, in performance of the Private Water Permit Program in accordance with the Illinois Water Well Construction Code.

Grantees must submit well permit applications and fees to IDPH upon receipt. IDPH reviews permit applications. Grantees must maintain documentation of activities conducted for a minimum of 5 years
