Short Description
Tobacco Settlement Recovery Funds and federal grants funds have been earmarked for the Department to provide educational outreach and enrollment/application assistance on Medicare Part D drug plans, Social Security's Extra Help, Medicare Savings Programs, Seniors Free Transit Ride, Persons with Disabilities Free Transit Ride, and the Secretary of State's License Plate Discount Program through the network of Area Agencies on Aging and local service providers. The Area Agencies on Aging have funded approximately 150 Senior Information and Assistance Sites around the state to help older adults and family members learn about and apply for these federal and state public benefits. The Legislative mandate of the Senior Health Assistance Program (SHAP) is to provide information and assistance to older adults about available pharmaceutical assistance programs.

Objective
The Senior Health Assistance Program will assist older adults to maintain their health by linking them to Medicare Part D drug plans, Social Security’s Extra Help, Medicare Savings Programs. The Social Security’s Extra Help can save eligible older adults an average of $3,900. It helps pay for Medicare Part D monthly premiums, annual deductibles, and prescription co-payments related to a Medicare prescription drug plan. Additionally, Medical Savings Programs can assist eligible older adults to save up to $1,258 per year on the costs of their Medicare Part B premiums. Thus, the use of the funds are critical to the life, health and safety of older citizens of Illinois. The Senior Health Assistance Program will assist older adults in maintaining their health, thereby preventing more expensive medical treatments, such as hospitalization. Thus, SHAP funds are in the best interests of the State of Illinois.

UGA Program Terms
1. The Area Agency on Aging agrees to administer the Senior Health Assistance Program in accordance with all applicable regulations, policies and procedures established by the Illinois Department on Aging. 2. The Area Agency on Aging must submit to the State Agency, for prior approval, any proposed contracts with profit making organizations to provide services under the Senior Health Assistance Program. 3. The Area Agency on Aging will have the ability to develop a Senior Health Assistance Program plan and to carry out, directly or through contractual or other arrangements, a program in accordance with the plan within the planning and service area. 4. The Area Agency on Aging will conduct outreach efforts to identify older individuals eligible for assistance under the Senior Health Assistance Program, with special emphasis on rural elderly, older individuals with greatest economic need (with particular attention to low-income minority individuals and older individuals residing in rural areas), older individuals with greatest social need (with particular attention to low-income minority individuals and older individuals residing in rural areas), older individuals with severe disabilities, older individuals with limited English proficiency, older individuals with Alzheimer’s disease and related disorders with neurological and organic brain dysfunction (and the caretakers of such individuals), and older individuals at risk for institutional placement; and inform such individuals of the availability of services under the Senior Health Assistance Program. 5. The Area Agency on Aging will ensure that each activity undertaken by the agency, including planning, advocacy, and systems development, includes a focus on the needs of low-income minority older individuals and older individuals residing in rural areas. 6. The Area Agency on Aging will coordinate planning, identification, assessment of needs, and provision of services for older individuals with disabilities, and individuals at risk for institutional placement, with particular attention to individuals with severe disabilities, with agencies that develop or provide services for individuals with disabilities. 7. The Area Agency on Aging has established and is following methods to assure that the Area Agency will serve as the advocate and focal point for older individuals within the community by monitoring, evaluating, and commenting on policies and programs, hearings, levies, and community actions which will affect older persons. 8. The Area Agency on Aging will provide a grievance procedure for older individuals who are dissatisfied with or denied services under the Senior Health Assistance Program. 9. The Area Agency on Aging assures that funds received under the Senior Health Assistance Program will be used to provide, to the extent feasible, for the furnishing of services under the Senior Health Assistance Program, consistent with self-directed care. 10. The Area Agency on Aging will assure that no officer, employee, or other representative of the Area Agency on Aging is subject to a conflict of interest prohibited under the Older Americans Act and mechanisms are in place to identify and remove conflicts of interest prohibited under the Older Americans Act. 11. The Area Agency on Aging has established and is following methods that ensure that all services provided through the Senior Health Assistance Program (SHAP) are provided without the use of any means tests.

Audits
Non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503.
