PROGRAM INFORMATION
Short Description
The Long Term Care Systems Development Grant (LTCSDG) funds are awarded to the thirteen Area Agencies on Aging (AAAs). These funds are used by the AAAs to support the Department on Aging’s Dementia Friendly Initiative as outlined in the State Plan on Aging, FY 2022-2024. During the planning stages, the AAAs should report a minimum of one meeting or activity per month. AAAs will partner with the Illinois Cognitive Resources Network (ICRN) who is the state lead on this initiative. In order to achieve the goal of the targeted community being listed on the national registry of Dementia Friendly communities, AAAs should utilize the four-phase implementation method as outlined at dfamerica.org.
The Department on Aging, through the LTCSDG, requires the AAAs to provide assistance with operational activities and management issues; and assistance with departmental planning and development activities in their respective Planning and Service Areas in order to coordinate and serve older persons at risk of institutionalization. Planning activities should include the creation of a comprehensive coordination of services under ADRD services and Social Isolation services. In addition, AAAs should develop ways to collaborate and train the Managed Care Organizations about other services available under the Older Americans Act. Finally, it is expected that the AAAs update the AgingIS information portal monthly to ensure that the information available to the public is accurate and consistent.
The Department is also requiring each AAA to be prepared to provide assistance to the Department with the Aging & Disability Resource Center (ADRC) Statewide Initiative (e.g., serving on committees and attending retreats) and to develop and implement ADRCs in the Planning and Service Area.

Objective
Long Term Care Systems Development grant funds are are awarded to the thirteen Area Agencies on Aging (AAAs) to achieve the following objectives:
A. Create Dementia Friendly Communities in each PSA
B. assistance with departmental planning and development activities, as requested by IDoA.
C. development of a No Wrong Door/ADRN system in the Planning and Service Area.
UGA Program Terms
1. The Area Agency on Aging agrees to administer the Long Term Care Systems Development grant in accordance with all applicable regulations, policies and procedures established by the Illinois Department on Aging.
2. The Area Agency on Aging accepts final financial and administrative responsibility for the conduct of the Long Term Care Systems Development grant.
3. The Area Agency on Aging will assure that no officer, employee, or other representative of the Area Agency on Aging is subject to a conflict of interest prohibited under the Older Americans Act and mechanisms are in place to identify and remove conflicts of interest prohibited under the Older Americans Act.

Audits
Non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503. In accordance with the provisions of Subpart F-Audit Requirements, under 45 CFR Part 75.500, nonfederal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year. Except as noted in 45 CFR Part 75.500
