Objective
Meet or exceed the FY16 Long-Term Care Ombudsman Program Benchmarks The Illinois Long-term Care Ombudsman Program benchmarks are a minimum requirement of activities to be performed by the Regional Ombudsman Programs. The Illinois benchmarks were set after completion of a limited comparison of state LTCOP benchmark measures by the National Ombudsman Resource Center on behalf of the Illinois LTCOP. The Office used the recommendations for future continuous quality improvements from the July 2013 benchmark report findings as the basis for creating the current benchmarks for the Illinois LTCOP. The following benchmarks are the minimum requirements for each Regional Ombudsman Program: Closed Cases - 43 Closed Cases for every 1000 beds. Consultations to Individuals - 135 Consultations to Individuals for every 1000 beds. Facility Staff In-Services - 1 in-service for every 20 long-term care facilities. - Long-term care facilities include: NFs, ALFs, SLFs, Shelter Care, ICF-DD - Follow federal guidelines for entering into the database: - multi-facility staff in-services count as 1 in-service - in-services held at the same facility within the same day in order to catch various shifts count as 1 in-service. Community Education Sessions - 12 Community Ed Sessions plus 1 additional session for each benchmark required FTE. - this includes 4 MFP Community Ed sessions + 4 HCO Community Ed sessions - i.e. If a program is to have an FTE = 5, the program would be expected to do 17 Community Ed sessions within the year. Resident Council Meetings - attend at least 1 RC Meeting at a minimum of 85% of the nursing facilities within the program area. - based solely on the number of nursing facilities (not AL, SLF, Shelter Care, ICF-DD) - a program with 100 NFs would need to attend a minimum of 85 resident council meetings FTE Paid Staff - maintain the highest FTE level within the past 3 years.

1. The Area Agency on Aging agrees to administer the Long-Term Care Provider Fund in accordance with all applicable regulations, policies and procedures established by the Illinois Department on Aging. 2. The Area Agency on Aging may not require the Regional LTC Ombudsman Programs to budget and develop matching funds for the awarded Long-Term Care Provider grant funds. 3. The Long-Term Care Provider funds have been allocated to expand Regional LTC Ombudsman Program Services; therefore, the Long-Term Care Provider grant funds may not be used to supplant existing Title III, Title VII, General Revenue Funds, and local funds allocated to the Regional L:TC Ombudsman Programs. Additionally, Long Term Care Provider funds cannot be used to match Area Plan-related funds that support the Regional LTC Ombudsman Program or other Area Plan-related programs. 4. The Area Agency on Aging can only retain a maximum of 2% (subject to the Department's approval of the Area Agency's submitted application) of the LTC Provider funds allocated for the PSA for Direct Advocacy Program activities. If the Area Agency on Aging retains any of the LTC Provider funds for Direct Advocacy Program activities, the amount of Direct Advocacy Program activities must be in proportion to the amount of LTC Provider funds retained by the Area Agency on Aging.

These funds shall be used for direct service staff to conduct required Regional Ombudsman Program service activities and to pay for related allowable Regional Ombudsman Program expenditures such as travel, training, and IT support.

Audits
Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor's Office of Management and Budget. See 30 ILCS 708/65(c); 44111. Ad min. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and,supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless a different retention period Is specified In 2 CFR 200.334 or 44111. Admin. Code 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all lltlgation, claims or audit exceptions involving the records have been resolved and final action taken.

Program Accomplishments
The Illinois Department on Aging (Department or IDoA) establlshed and operates the Office of the State Long-Term Care Ombudsman Program (SLTCOP). Regional LTCOP services are delivered through provider agencies by individuals designated by the State Long-Term Care Ombudsman and are operated through a grant between the Department and Area Agencies on Aging. AAAs provide administrative and advocacy support, recommend Regional LTCOP provider agencies to the SLTCOP for designation, ensure Regional LTCOP services are delivered in accordance with applicable laws, rutes, regulatfons, a State plan, the LTCOP Policies and Procedures, an application for the provision of LTCOP services, benchmark directives set by the SLTCOP, a notice of award, a Corrective Action Plan Imposed by the SLTCOP, and the grant agreement between the Department and AAAs, provide support, and conduct legislative outreach to advance resident rights. The Long-Term Care Ombudsman Program works to protect and promote the rights and quality of life for long-term care residents, focusing on ensuring that existing state and federal laws as well as rules and regulations are adhered to and that resident and family voices are heard during drafting or revision of laws or rules through the advocacy service components of the program.
