Objective
To provide nutritious meals (breakfast, lunch, and/or supplements) to seniors at the contracted Community Care Program Adult Day Service sites in Illinois through the Child and Adult Care Food Program funded by the Untied States Department of Agriculture.

UGA Program Terms
1. The Service Provider agrees to administer the Child and Adult Care Food Program (CACFP) under the Federal Register, Department of Agriculture, Food and Nutrition Service 7 CFR Part 226 and as regulated under the Community Care Program/Adult Day Care Program. 2. The Service Provider agrees to ensure meals served meet the requirements and the provisions set forth by Department of Agriculture in 7 CFR Part 226, are in accordance with Child and Adult Care Food Program (CACFP) rules and serve the same meals to all adults (unless otherwise specified on the Enrollment Form or in the enrolled adult’s plan of care. 3. The Service Provider agrees to administer the Child and Adult Care Food Program (CACFP), specifically the meals and/or supplement services declared on the Income Eligibility Form to all adults enrolled in the Community Care Program at the Adult Day Care center. All enrolled adults will be identified and designated to qualify under an authorized line of eligibility as a free, reduced, or private-pay entity. 4. The Service Provider agrees to ensure no physical segregation, or discrimination, is made against any adult because of his/her inability to pay the full price of meals. 5. The Service Provider shall ensure reimbursement is claimed for no more than two (2) meals and one (1) supplement or two (2) supplements and one (1) meal served to enrolled adults at approved meal sites in accordance with Child and Adult Care Food Program (CACFP) rules and regulations. 6. The Service Provider shall ensure adequate supervisory and operational personnel are provided for overall monitoring and management of the Child and Adult Care Food Program (CACFP) at the food service site(s). The Service Provider must ensure that adequate personnel visit all sites under the program, take prompt action to correct deficiencies found at the time of the initial on-site visit, and review food service operations at every site in accordance with 7 CFR Part 226. 7. The Service Provider agrees to submit a Monthly Invoice Report for meal/supplement reimbursement in the web-based Child and Adult Care Food Program (CACFP) System and/or hard-copy version. Rates of Reimbursement provided by the United States Department of Agriculture (USDA)/Child and Adult Care Food Program (CACFP), are based on the number of eligible breakfasts, lunches, and snacks in the Service Provider’s monthly reports. Payment provisions are in accordance with 7 CFR 226, Subpart D and procedures established by the Illinois Department on Aging. 8. The Service Provider agrees to comply with all federal and state financial reporting requirements as may be necessary to resolve discrepancies in billing, to verify costs for determining reimbursement rates, and to permit fiscal monitoring by the Department including financial reporting requirements. Falsification or concealment of a material fact in regards to service provided or billing may lead to further legal action, up to and including negative certification action and contract termination.

Public nonprofit institution/organization (includes institutions of higher education and hospitals), U.S. Territories and possessions, U.S. Territories and possessions, Private nonprofit institution/organization (includes hospitals with an active Community Care Program Adult Day Service contract).
Beneficiary Eligibility
Approved institutions providing non-residential day care services, including Adult Day Service sites may participate in CACFP. Eligible public and non-profit private organizations may include adult day care centers. Private for-profit centers may also participate if at and at least 25% of the Adults enrolled in the Adult Day Service center receive benefits under Title XIX, Title XX, or a combination of both. Any eligible institution may participate in CACFP upon request with State Agency approval, but must have an active Community Care Program Adult Day Service contract in Illinois to be considered/approved by the State Agency.

Applicant organizations must furnish evidence of nonprofit status. Costs will be determined in accordance with USDA Uniform Federal Assistance Regulations. 2 CFR 200, Subpart E - Cost Principles applies to this program.
Preapplication Coordination
Preapplication coordination is required. Environmental impact information is not required for this program. This program is eligible for coverage under E.O. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in his or her State for more information on the process the State requires to be followed in applying for assistance, if the State has selected the program for review. At the local level, households may apply for free or reduced price meals under the lunch program by submitting an application furnished by the Food and Nutrition Service (FNS), the State Agency, or the School Food Authority, as applicable.

2 CFR 200, Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards applies to this program. Appropriate forms may be obtained from the administering State Agency.
Criteria Selecting Proposals
Eligibility for an application/proposal for enrollment in the CACFP program (adult component) is determined if a existing Community Care Program Adult Day Service contract is in good standing (as there is no licensure of Adult Day Service centers in Illinois at this time). Meeting this requirement, applications are reviewed by the State Agency based on the Adult Day Service organization's ability to comply with the perimeters as set forth in the CACFP application, hence the SDA Agreement.
Award Procedures
The State Agency reviews the written application submitted by an Adult Day Service organization, upon determination of eligibility, makes a written agreement (SDA) with the Adult Day Service organization or institution for participation in the CACFP program.

Formula Matching Requirements
Statutory Formula: Title Chapter Part Subpart Public Law Matching Requirements: Percent: 30 As specified in Section 7 of the National School Lunch Act, State revenue matching must be not be less than 30 percent of the funds received by the State agency under section 4 of the National School Lunch Act beginning in the 1980-81 school year. Uses and Restrictions
Funds can be used for the administration of nutritious meals/supplements provided to the adult individual in the Adult Day Service center setting and only in a remote setting (as waived) for the duration of a Federally declared public health emergency.
Reports
Program Reports: State agencies and Adult Day Service organizations file monthly reports on the number of meals served, by type, to claim program funds. Adult Day Service organizations food authorities must submit final meal claims to the State Agency no later than 60 days after the claiming month, and State Agency must submit final program reports to FNS no later than 90 days after the claiming month. The State Agency must submit final fiscal year grant close-out reports to FNS within 120 days after the end of the fiscal year to which they pertain. Annually, food authorities must report, to their State Agency, the most common charge for lunches served to adults who do not qualify for free or reduced price lunches. Food authorities report the price charged at the Adult Day Service organization level, as applicable. In turn the State Agency submits this information to FNS for publication. Expenditure Reports: Reports are required as specified in 7 CFR 210.5(d).
Audits
In accordance with the provisions of 2 CFR 200, Subpart F - Audit Requirements, nonfederal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503 Audits may be conducted less frequently under conditions specified in OMB Circular No. A-133.
Records
Adult Day Service organization food authorities must maintain full and accurate records of their nonprofit food service operation, including --but not limited to-- records pertaining to finances, the price of paid lunches in relation to food costs, menu planning and production, and meal counting and claiming. Adult Day Service organization food authorities must make these records available to the State Agency for review.

Program Accomplishments
The CACFP program provides daily nutritious lunches and supplements to low income and the most vulnerable adult population (60 years and older) through the State Agency, hence the Adult Day Care program in Illinois.
