Applicant Eligibility
Agency has qualified under the Internal Revenue Code Section 501 (c) 3 and/or 511 and serves as their own fiscal agent or the CCR&R’s parent agency has a 501 (c) 3 status and serves as the CCR&R’s fiscal agent. Access to computerized systems provided by the Illinois Department of Human Services (IDHS) to verify eligibility information, enter payments, monitor training, and licensing status are mandatory to successful contract fulfillment. Contracted agencies must have high speed Internet access to work with IDHS web-based computerized systems. Minimum hardware requirements include computers, printers, scanners and copiers. Software requirements include Adobe Acrobat and Microsoft Office Professional. Although various computer Operating Systems may work with IDHS systems, agencies must have an operating system that can interface with Internet Explorer 8 or above (IE8). IDHS only approves access using Internet Explorer. Other browsers that IDHS does not support may have technical difficulties. Agencies must secure an approval from the Department of Innovation and Technology (DoIT) on their Security and Privacy Controls Questionnaire (SPCQ) prior to obtaining eligibility systems access.

Preapplication Coordination
Each applicant must have access to the internet. The Department's website will contain information regarding the NOFO and materials necessary for submission. t is the responsibility of each applicant to monitor the DHS web site and comply with any instructions or requirements related to the NOFO. Each applicant will be required to submit a Uniform Application for State Grant Assistance. The application template and all related requirement for format and content may be found on the Illinois Department of Human Services website. Additionally, applicants are required to submit a Budget, Budget Narrative and Program Plan. The application must be submitted electronically using the provided templates. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR §25.110 (b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR §25.110 (d)) is required to: • Be registered in SAM before submitting its application. If you are not registered in SAM, this link provides a connection for SAM registration: https:(/www.sam.gov/portal/SAM/#1 ; • provide a valid DUNS number in its application; and o continue to maintain an active SAM registration with current information at all times during which it has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency. The State awarding agency may not make a Federal pass-through or State award to an applicant until the applicant has complied with all applicable DUNS and SAM requirements and, if an applicant has not fully complied with the requirements by the time the State awarding agency is ready to make a Federal pass-through or State award, the State awarding agency may determine that the applicant is not qualified to receive a Federal pass-through or State award an use that determination as a basis for making a Federal pass-through or State award to another applicant. Applications must be received at the location below no later than 12:00 p.m., on June 16, 2016. The application container will be time-stamped upon receipt. The Department will ONLY accept applications submitted by electronic mail. Applications will NOT be accepted if received by fax machine, hard copy, disk or thumb drive. Applications will be opened as they are received. Applicants will receive an email (within 72 hours of receipt or 120 hours if received on a non-business day) notifying them that their application was received and if it was received by the due date and time. This email reply will be seen to the original sender of the application. Applications received after the due date and time will not be considered for review or funding. To be considered, applications must be emailed by the designated date and time noted above. Please be sure to keep a copy for your records, including a copy of the email submission with the date and time the application was submitted along with the email address to which it was sent. The deadline will be strictly enforced. In the event of a dispute, the applicant bears the burden of proof that the application was received on time at the location noted above. All applications must include the following mandatory forms/attachments: • Uniform Application for State Grant Assistance (template provided) • Program Plan (template obtained from Cindy Wall at the e-mail address provided above) • Accessibility Plan (template obtained from Cindy Wall at the e-mail address provided above) • Uniform Budget (template obtained from Cindy Wall at the e-mail address provided above) • Copy of Currently Approved NICRA if indirect costs are included in the budget All applicants are subject to the funding restrictions relating to allowable and unallowable costs as set forth in 2 CFR 200, Subpart E - Cost Principles. Additionally, unallowable costs include, but are not limited to the following: • Bad debts • Contingencies or provision for unforeseen events • Contributions or donations • Entertainment, alcoholic beverages, gratuities • Fines and penalties • Certain interest and financial costs • Legislative and lobbying expenses • Real property payments or purchases Additionally, equipment purchases must be preapproved and will be evaluated during the budget process. Funding for the period of January 01, 2017 through June 30, 2017 is not guaranteed. All applicants must demonstrate that they meet all requirements under this NOFO described throughout. Applications that fail to meet the criteria described in the "Eligible Applicants" will not be scored nor considered for funding. The Department will follow the Merit-based review process established by the Governor's Office of Management and Budget. Applicants recommended for funding under this NOFO following the above review and selection process will receive a "Notice of State Award Finalist". This notice will identify additional grant award requirements that must be met before a grant award can be executed. These will include one or more of the following: • Grantee Pre-Qualification Process • Financial and Administrative Risk Assessment -Internal Control Questionnaire • Conflict of Interest and Mandatory Disclosures A Notice of State Award (NOSA) will be issued to the review finalists that have successfully completed all grant award requirements. Based on the NOSA, the review finalist is positioned to make an informed decision to accept the grant award. The NOSA shall include: • The terms and conditions of the award. • Specific conditions assigned to the grantee based on the fiscal and administrative and programmatic risk assessments. Upon acceptance of the grant award, announcement of the grant award shall be published by the awarding agency to Grants@Illinois.gov. The NOSA must be signed by the grants officer (or equivalent). This signature effectively accepts the state award and all conditions set forth within the notice. This signed NOSA is the authorizing document. The Agency signed NOSA must be remitted to the Department as instructed in the notice. The agency awarded funds shall provide services as set forth in the OHS grant agreement and shall act in accordance with all state and federal statutes and administrative rules applicable to the provision of the services. CR&R agencies serve all parents and families within the defined Service Delivery Area (SDA). n addition, CCR&R agencies work with child care providers, their communities and policy makers. Services provided by the CCR&R agency shall include: 1. Administration of the Child Care Assistance Program (CCAP) a. CCR&R shall administer the Child Care Assistance Program (CCAP) on behalf of the Department to determine family eligibility and process child care provider payments for those eligible families. This includes, but is not limited to, offering information on the CCAP, the initial and on-going eligibility determination processes, payment process, on-going case management, case problem resolution actions, case status and action research (as requested), community education and outreach. 2. Assemble and maintain a child care provider database 3. Consumer education and child care referrals to parents/guardians a. Offering information on child care options, quality care indicators, costs, and referrals to child care providers 4. Training and technical assistance to child care providers a. Offering technical assistance, training and consultation relevant to the early childhood field and to provide quality child care services b. Offering basic information on other support services, specifically publically funded services 5. Recruitment and retention activities for child care options a. CCR&R shall develop plans for addressing shortages of child care services in communities with high concentration of persons eligible for, or receiving, child care assistance. Such plans will address the needs of persons in these communities who are participating in approved education and/or training programs as well as the needs of those persons who are employed. 6. Data collection and analysis a. Providing comprehensive data on child care supply and demand 7. Community and Provider Services Programs a. Community Systems Development is designed to connect and coordinate programs and services for young children and their families. Participate in and/ or assist community collaboration groups to provide coordinating services for families. b. Provider Service Programs are designed to enhance the professional development of individual child care practitioners, improve program quality, improve the quality of care given to children and assist providers to achieve and maintain an ExceleRate Illinois Circle of Quality
Reports
The provider will submit the following reports in the format prescribed by the Department and within the established time frames. A. Program plans - reports shall be submitted on a quarterly bases no later than 15 days following the end of each quarter B. Expenditure reports - monthly expenditure reports are due no later than 15 days after the end of each month C. Program Data Reporting - the CCR&R shall enter and submit program data in the statewide system no later than ten (10) days after the end of each quarter. Additional data may be collected as directed by the Department and in a format prescribed by the Department.
Audits
Awardees, excluding for-profits, that expend $750,000 or more during the non-federal entity's fiscal year in federal awards (federal pass-through and direct federal funds) must have a single audit conducted in accordance with UR section 200.514.
