Short Description
Project to transition individuals age 60 and older, individuals with physical disabilities, and individuals with intellectual and developmental disabilities (I/DD) from institutional to community-based settings. The Money Follows the Person Program (MFP) in Illinois ended 12/31/2017. It had stopped accepting referrals as of 6/30/2017. IL’s demonstration has been officially closed with federal CMS. A number of states do still operate the MFP Program; however, IL chose to end its participation when it moved towards engaging more fully in Managed Care. In 2021, the federal MFP program invited states with no current MFP program to apply for a Planning Phase grant to build or rebuild the structure needed for MFP implementation. Since the program aligns with IL's overall goals of increasing the use of community services and rebalancing the state's long term care systems by providing appropriate, person-centered services for individuals interested in transitioning from nursing facilities and other institutional settings to qualified home and community-based settings, IL applied for and received funding to plan for MFP reimplementation.

Objective
The Money Follows the Person Program (MFP) seeks to increase the use of home and community based services (HCBS) and reduce the use of institutionally-based services; eliminate barriers and mechanisms in State law, State Medicaid plans, or State budgets that prevent or restrict the flexible use of Medicaid funds to enable Medicaid-eligible individuals to receive long-term care in the settings of their choice; strengthen the ability of Medicaid programs to assure continued provision of HCBS to those individuals who choose to transition from institutions; and ensure that procedures are in place to provide quality assurance and continuous quality improvement of HCBS.

The initial MFP program, referred to as Pathways to Community Living, was available to individuals aged 60 and older; individuals with physical disabilities; individuals with mental illness; and individuals with intellectual/developmental disabilities (I/DD). The individual also needed to reside in a long-term care facility (nursing home) or Intermediate Care Facility for Individuals with Developmental Disabilities (ICF/DD) for a minimum of 90 days, be currently receiving or be eligible for Medicaid, be interested in moving back to the community, and be capable of having their needs met appropriately and safely within the community. The current MFP program includes many of the same requirements, with the exceptions of (1) the applicable populations include individuals 60 and older, individuals with physical disabilities, and individuals with I/DD, the 90 days within an institutional setting has been reduced to 60 days, and there are enhanced opportunities for funding of services not typically covered by Medicaid, such as short-term housing assistance, pantry supplies, and home furnishings.

The new MFP program is available to individuals who are over age 60; individuals with physical disabilities; individuals with intellectual/developmental disabilities (I/DD). The individual must also currently reside in a long-term care facility (nursing home) or Intermediate Care Facility for Individuals with Developmental Disabilities (ICF/DD). To be considered for this program, an individual must have resided in the facility for at least 60 days, be currently receiving or eligible for Medicaid, be interested in moving back to the community and choose to do so and be able to have their needs met appropriately and safely in the community.
