Objective
Reduce infant and maternal mortality; perform assessments of individual/family needs, provide linkage with Medicaid/primary care, refer for assistance with identified social needs, & coordinate care through face-to-face contacts and home visits at regular intervals throughout pregnancy and the infant's first year of life./ Minimize disability in high-risk infants by early identification of possible conditions requiring further evaluation, diagnosis, and treatment; Promote optimal growth and development of infants; Teach family care of the high-risk infant; Decrease stress and potential for abuse.

Pregnant & Postpartum women, and infants below 200% of the Federal Poverty Level
Infants and children (ages 0 – 2 years old) are referred to the high-risk infant follow up program either through the IDPH Adverse Pregnancy Outcomes Reporting System (APORS) or based on assessments done in the Family Case Management program which determine: that the infant has been diagnosed with a serious medical condition after newborn discharge; when maternal alcohol or drug addiction has been diagnosed; or when child abuse or neglect has been indicated based on investigation by the Illinois Department of Children and Family Services (See 410 ILCS 525/3 and 77 Ill. Adm. Code 840.210).

In accordance with GATA Administrative Rules, Section 350, Merit Based Review of Grant Applications, a merit-based application review is required for competitive Grants and Cooperative Agreements, unless prohibited by State or Federal statute.

Uses and Restrictions
Allowable Costs
Activities performed as necessary to meet Program objectives may be billed to the grant and provided all other requirements for allowability are satisfied, the direct and indirect costs associated with performing these activities are allowable charges to the grant.
a) Participant enrollment including data collection and assessment for care plan development, health education and anticipatory guidance, referrals and follow up with medical home.
b) Program management activities including accounting, auditing, budgeting and outreach.
All uses must comply with Part 200 Federal Uniform Guidance and Program Policy and Procedure Manual;
Reports
Project Grants require quarterly progress and financial reports and monthly expenditure reports on operating expenses and funding.

Records
Each Local Agency shall maintain full and complete records of Program operations in compliance with Federal and State records retention requirements. All records shall be retained for three (3) years following the close of the fiscal year to which the records pertain.
An agency (e.g., any court and all parts, boards, departments, bureaus, and commissions of any county, municipal corporation or political subdivision) shall comply with the Local Records Act, which regulates the destruction and preservation of public records within the State of Illinois.
