The goal of the Healthcare Transformation Collaboratives is to reorient our healthcare delivery system around people and communities. There are four major components to the Transformation plan: (1) focusing on community needs, for all levels of healthcare, with an emphasis on addressing social and structural determinants of health; (2) improving health and wellness for individuals and communities; (3) tailoring solutions to meet the unique needs of individual communities; and (4) investing in projects, large and small, that improve outcomes, decrease disparities, and are sustainable over time. The HTC program will award funds exclusively to collaborations -- across healthcare systems, community organizations, community residents and other community-based entities. HTC funds are not grants, but rather direct payments to beneficiaries through Medicaid. Accordingly, collaborations that receive awards must include at least one entity that bills Medicaid.

Objective
It is the intention of the General Assembly that innovative partnerships funded must be designed to establish or improve integrated health care delivery systems that will provide significant access to the Medicaid and uninsured populations in their communities, as well as improve health care equity. It is also the intention of the General Assembly that partnerships recognize and address the disparities revealed by the COVID-19 pandemic, as well as the need for post-COVID care

The HTC funding is available to collaborations between care providers, including preventative care, primary care, specialty care, hospital services, mental health and substance abuse services, and community- based entities that address the social determinants of health. Collaborations must include at least one Medicaid Provider that is eligible to bill Illinois’ Medicaid Program

Criteria Selecting Proposals
The key requirements to be eligible for Healthcare Transformation Collaboratives funding include: •Demonstrate that the proposed project is a collaboration of providers coming together to improve care and access for community members the proposed project addresses community needs, and desires established via direct community input •Demonstrate that the proposed project addresses community needs and desires established through direct community input •Provide data that supports the need for the proposed project •Include at least one Medicaid-enrolled provider eligible to bill for Medicaid services in the collaboration structure •Propose quality metrics to track the impact of the proposed intervention(s) and for which the project will be accountable •Incorporate racial equity into the proposed interventions and collaboration •Propose interventions that maintain or improve current job levels in the community (proposals that include retraining, innovative ideas or other workforce development are of particular interest to HFS) •Demonstrate how an initial project funded by HFS through this program will move to a place of financial sustainability in the future without subsidization by Transformation funds •Demonstrate that collaborators are governed by a structure that clearly shows how decisions will be made, how collaborators will be held accountable for their work on the project and how collaborators will monitor and enforce adherence to the policies and practices put in place.

Uses and Restrictions
• Collaborations must include at least one Medicaid Provider that is eligible to bill Illinois’ Medicaid Program. • Priority will be given to collaborations that include safety net hospitals or critical access hospitals, as well as minority controlled or led organizations • Funds will be distributed as enhanced directed payments to some or all collaborative members as payment for Medicaid eligible services in order to ensure the funds are eligible for federal match. • funds shall be expended by the Department in a manner that maximizes federal funding to expend the entire allocated amount. • No disbursement of moneys for transformation projects from the transformation funding pool described under this Section shall be considered an award, a grant, or an expenditure of grant funds. • Funding agreements made in accordance with the transformation program shall be considered purchases of care under the Illinois Procurement Code. • Purchase of care: "Purchase of care" means a contract with a person for the furnishing of medical, educational, psychiatric, vocational, rehabilitative, social, or human services directly to a recipient of a State aid program (30 ILCS 500/1-15.68- Sec. 1-15.68).
