Objective
The purpose of this grant is to continue and expand the Illinois Care Connections Program to support older adults aged 60 and older by providing technology and/or assistive devices that enable social connectedness; maintain health, safety, and wellbeing; and promote independence and community living.

Nonprofit Organizations; For-Profit Organizations; Government Organizations; Education Organizations;

Applicant must be the Illinois Assistive Technology Program
Beneficiary Eligibility
Beneficiary must be an older adult, age sixty (60) or older.

Reports
Monthly and quarterly progress reports and final annual reports are required of grantees. Performance monitoring accomplished through calls, site visits, review of progress reports, and general interaction between the Program Officer and grantee.
