Direct services to be provided include, but are not limited to: - Education and counseling on HIV and STD prevention to clients during comprehensive and problem visits. - Screenings for chlamydia and gonorrhea of sexually active clients less than 25 years of age at initial and annual exams, during pregnancy tests, and at appropriate problem visit. - Preconception education and counseling at new and annual visits including reproductive health plan - Treatment of chlamydia and gonorrhea within 30 days of diagnosis. - Review contraceptive methods of female clients during all visit types and provide education, counseling, and access to chosen method. Outreach/Education to be provided: - Provide community education and outreach to target population. Increase community outreach affiliations with neighboring partners and schools.

Applicants must submit a completed Universal Grant Application for FY21 as well as complete the pre-qualification processes. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: be registered in SAM before submitting the application; provide a valid DUNS number in its application; and continue to maintain an active SAM registration with current information at all times during which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency.

Uses and Restrictions
Allowable Costs Activities performed as necessary to meet Program objectives may be billed to the grant and provided all other requirements for allowability are satisfied, the direct and indirect costs associated with performing these activities are allowable charges to the grant. a) Participant enrollment including data collection and assessment for care plan development, health education and anticipatory guidance, referrals and follow up with medical home. b) Program management activities including accounting, auditing, budgeting and outreach. All uses must comply with Part 200 Federal Uniform Guidance and Program Policy and Procedure Manual;

Project Grants require quarterly progress and financial reports and monthly expenditure reports on operating expenses and funding.

Records Each Local Agency shall maintain full and complete records of Program operations in compliance with Federal and State records retention requirements. All records shall be retained for three (3) years following the close of the fiscal year to which the records pertain. An agency (e.g., any court and all parts, boards, departments, bureaus, and commissions of any county, municipal corporation or political subdivision) shall comply with the Local Records Act, which regulates the destruction and preservation of public records within the State of Illinois.
