Short Description
Addressing disparities in public health that have been exacerbated by the Covid-19 pandemic through education, screenings, and vaccinations at community health fairs and vaccination clinics. Provide funding for mammograms when all other means of payment have been exhausted.

Objective
Services to be provided include, but are not limited to: - Host at least two (2) community health fair events to provide health education, screenings, and Covid-19 vaccinations - Host at least one (1) Covid vaccination clinic at Cook County Jail in collaboration with Cook County Health Department to provide health education, screenings, and Covid-19 vaccinations - Host at least one (1) Covid vaccination clinic at Peoria County Jail in collaboration with Cook County Health Department to provide health education, screenings, and Covid-19 vaccinations - Provide reimbursement to Unity Point for at least 200 completed mammograms for individuals who have exhausted all other means of payment

Preapplication Coordination
Applicants must submit a completed Universal Grant Application for FY21 as well as complete the pre-qualification processes. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: be registered in SAM before submitting the application; provide a valid DUNS number in its application; and continue to maintain an active SAM registration with current information at all times during which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency.

Uses and Restrictions
The Grant Funds awarded pursuant to this Agreement are a subaward from the State’s allocation from the Coronavirus State Fiscal Recovery Fund ("CSFRF") established by Section 602 of the Social Security Act, 42 USC 802. Grantee is required to comply with, and is subject to, all requirements of the CSFRF and all related rules and guidance issued by Grantor and the U.S. Department of the Treasury Allowable Costs Activities performed as necessary to meet Program objectives may be billed to the grant and provided all other requirements for allowability are satisfied, the direct and indirect costs associated with performing these activities are allowable charges to the grant. a) Participant enrollment including data collection and assessment for care plan development, health education and anticipatory guidance, referrals and follow up with medical home. b) Program management activities including accounting, auditing, budgeting and outreach. All uses must comply with Part 200 Federal Uniform Guidance and Program Policy and Procedure Manual;
Reports
Project Grants require quarterly progress and financial reports and monthly expenditure reports on operating expenses and funding.

Records
Records Each Local Agency shall maintain full and complete records of Program operations in compliance with Federal and State records retention requirements. All records shall be retained for three (3) years following the close of the fiscal year to which the records pertain. An agency (e.g., any court and all parts, boards, departments, bureaus, and commissions of any county, municipal corporation or political subdivision) shall comply with the Local Records Act, which regulates the destruction and preservation of public records within the State of Illinois.
