Uses and Restrictions
By requesting or receiving funds under this award, the recipient assures that it will carry out the project/program described in its letter of assurance and will comply with the terms and conditions and other requirements of this award.
Reports
SF-425 Financial Reporting: Grantee is required to submit SF-425s on a semi-annual basis. The SF-425 shall be submitted using the HHS’ Payment Management System (PMS). PMS website is located at: https://pms.psc.gov. Reports are due within 30 days for the periods ending September 30 and March 31 (i.e., due October 30 and April 30), through September 30, 2022, a final PMS drawdown and a final SF-425 are due within 120 days after September 30, 2022 (i.e., due January 31, 2023). If a final SF-425 report will be submitted January 31, 2023, a semi-annual report is not required to be filed for report ending September 30, 2022. Quarterly reporting to IDoA using the PPR and PFR forms.
Audits
Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor's Office of Management and Budget. See 30 ILCS 708/65(c); 44111. Ad min. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, rmd all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44111. Ad min. Code§§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the recoras must Ue retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.

Retain staff and sustain programs within Adult Protective Services, increase the number of ESO's, monitor grants including document review, help sustain the network through staff retention, and add additional staffing to process payments more efficiently.
