Short Description
The purpose of this grant is to support the Grantee for the cost of TCARE license fees and/or support staffing costs that will allow grantee to train staff in TCARE usage and implementation. The funding was made possible by the Department’s participation in the National Academy of State Health Policy (NASHP) State Medicaid Policy Institute on Family Caregiving (Institute). The Department is providing this funding in order to meet the NASHP Institute goal to expand the utilization of TCARE by Area Agencies on Aging (AAAs) statewide. Using an assessment tool that is consistent across the state will produce consistent data and the ability to track outcomes. In the data driven environment, tracking outcomes is crucial to ensure interventions are effective and precise in solving the issues being targeted through a program such as Family Caregiver Support Program (FCSP). Having a standard assessment tool in which the same data points can be gathered across time demonstrate the cost-savings of caregiver support versus the cost of long term care, as well as many other metrics.

Reports
The grantee shall deliver to the Department two reports, utilizing the Periodic Performance and Periodic Financial Reports. The report should provide details about the number of staff hired and/or trained in the TCARE assessment tool. The report should also provide confirmation that a TCARE license(s) were purchased as a result of receipt of this grant. The reports shall be due as follows: The first report shall be for the period January 1, 2022 through June 30, 2022. This report shall be due to the Department on or before July 31, 2022. The second report shall be for the period July 1, 2022 through December 31, 2022. This report shall be due to the Department on or before January 30, 2023.
