Serves pregnant and parenting teens in Illinois’ 8th District by increasing access to healthier food and beverage options and nutritional educational opportunities in schools and community health centers to promote positive maternal and infant health outcomes.

Objective
I. Determine motivations/preferences and awareness/uptake of state-based, managed care organization-based, and community-based nutrition program and case management utilization in various communities within the Illinois’ 8th District: A. Convene in-person discussions with pregnant and parenting (within 12 months postpartum) teens between ages 13-19 who reside in the Illinois 8th District. B. Conduct stakeholder meetings with public school administrators, public health agencies, community-based health center case management and local nonprofit organizations. C. Analyze data of resident population who utilize IDHS, MCO, and non-state funded nutrition and case management programs. Population data subset to include age (restricted to 13-19 years old); race groups, payer groups and geographic groups. II. Increase nutrition education for the teen pregnant and parenting population within the Illinois’ 8th District food deserts. A. Collaborate with public schools with pregnant and parenting teens to identify opportunities to educate the student population on proper nutrition specific to pregnancy and postpartum periods. B. Collaborate with community health centers to provide nutritious foods and beverages for the teen pregnant and parenting population and encourage continuum of maternal and child health care.

Preapplication Coordination
Applicants must submit a completed Universal Grant Application for FY23 as well as complete the pre-qualification processes. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: be registered in SAM before submitting the application; provide a valid EUI number in its application; and continue to maintain an active SAM registration with current information at all times during which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency.
