Short Description
To ensure that a short-term Nursing Facility (NF) stay does not become a long-term stay, three Care Coordination Units (CCUs) covering the City of Chicago will identify 10 class members each, 30 in total, in collaboration with IDoA, DHS and UIC-CON will identify 10 class members each, 30 in total, who are Medicaid Fee-for-Service that request follow up by a Care Coordination Unit during the Choices for Care consultation. Each of the three CCUs will identify 10 class members utilizing the IDoA Service Selection and Certification form (IL – 402-1317) that is completed as part of the Choices for Care consultation. The Choices for Care process is required by State Law (Public Act 099-0857) and it requires CCUs to inform older adults, regardless of Medicaid status, of their community-based options in addition to institutional care options. This process allows class members the opportunity to request follow up at the NF, post hospitalization, and includes a section to collect phone numbers from the individual, family, and authorized representatives. Eligibility for the pilot will be based on the following: age 60 or above (consistent with the CCUs and IDoA’s mission), not a current Community Care Program Persons who Are Elderly Waiver participant, and residents who do not have a discharge plan at the time of admission to a nursing facility. Pilot program participants will be Medicaid Fee-For-Service and not have a primary diagnosis of SMI. Please note that any changes to salary, wage, insurance, or fringe benefits will not be supported by IDoA after the end of the grant period. Each CCU contract may request up to $333,000, with the final awarded amount being split across two fiscal years (FY23: November 01, 2022 – June 30, 2023); FY24: July 01, 2023 -October 31, 2023). As part of the application process, each agency shall submit a breakdown of funds requested each year. The maximum allocation per agency contract for FY23 is $200,000 with any funds below that amount being allowed for addition to an agency’s FY24 allocation. No agency’s direct cost to administer the grant will be permitted to exceed 20% of the total allocation for both awarded fiscal years.

Grant funds may be used for the following (but are not limited to): • To hire a Colbert -specific Care Coordinator who can devote 100% of their time to Colbert class during the pilot period. At the conclusion of the pilot period, the Department recommends maintaining the Care Coordinator through increased to Choices and/or CCP caseload. • Insurance/Fringe benefits for the Colbert – specific Care Coordinator • One- time expenses • Supporting or offsetting any costs related to Colbert class member assessment, care planning, transition planning, and identifying other non-waiver long-term services and supports. • Housing assistance (if other Colbert resources are not available in a timely fashion)
Reports
Quarterly financial and programmatic reports will be required. In addition, the grantee shall deliver to the Department a monthly expenditure report due on or before the 15th of each month and shall be inclusive of all expenses for the previous month for reimbursement should expenses reported go beyond cash on hand from previous payments made by the Department.
