Objective
Direct Services Oak Park River Forest Infant Welfare Society aims to increase access to medical, dental and behavioral health care for low-income Chicago area children and to improve the health status of Clinic patients. Services to be provided include, but are not limited to: - Provide free and low-cost dental care for publicly insured and uninsured children from west suburban Cook and DuPage Counties and the City of Chicago - Provide preventive dental care for low-income children in schools - Reduce the incidence of plaque in dental patients - Increase the proportion of patients who are up to date on their immunizations - Improve the identification and treatment of developmental, social, emotional and behavioral health needs and concerned - Provide oral health and nutrition education for low-income children through school partnerships - Provide community health clinical opportunities for medical and dental students - Provide quarterly progress report to DHS on progress towards the goals - Provide an annual report of OHF activities funded by the Department within thirty (30) days of the end of the fiscal year. -

Preapplication Coordination
Applicants must submit a completed Universal Grant Application for FY24 as well as complete the pre-qualification processes. Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: be registered in SAM before submitting the application; provide a valid EUI number in its application; and continue to maintain an active SAM registration with current information at all times during which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency.
