Short Description
CCA and CHaSCI will provide an evidence-based framework for enhancing the Choices for Care assessment in an effort to improve care transitions and deflect older adults from avoidable skilled nursing facility stays, in part by identifying, assessing, and addressing social determinant of health and caregiving needs. The demonstration project will utilize the Bridge Model of transitional care, Medicare Chronic Care Management Codes, and Illinois Department on Aging funding, while also leveraging the CARE (Caregiver Advise, Record, Enable) Act, to improve the care older adults receive while transitioning out of a hospital or skilled nursing facilities back into the community.

Uses and Restrictions
Funds made available under the General Revenue shall supplement, and not supplant, any Federal, State, or local funds expended by the Grantee to provide services described in their approved application. Reports
The grantee agrees to submit financial and programmatic reports as requested and in the format required by the grantor.
Audits
Grantee shall be subject to audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501- 7507) and Subpart F of 2 CFR Part 20 and the audit rules and policies set forth by the Governor's Office of Management and Budget.
