Short Description
As American and Illinois populations age, the prevalence of people living with Alzheimer’s Disease or related dementias (ADRD) increases as well. As of 2023, more than 6 million Americans age 65 and older are living with Alzheimer's dementia; seventy three percent of those individuals (73%) are age 75 or older. By 2050, it is projected that 12.7 million Americans 65 and older will be living with ADRD. In 2020, an estimated 230,000 Illinoisans age 65 and older are living with Alzheimer’s dementia. This number is expected to increase to 260,000 in 2025, representing a 13% prevalence increase in just a five year period . To better assist older adults living with ADRD to thrive within community settings and to support the needs of their caregivers, the Illinois Department on Aging (IDoA) is prioritizing participation in the Dementia Friendly America Initiative. Illinois was designated a Dementia Friendly State in 2017. IDoA continues to actively work to increase the capacity of the aging network in Illinois to respond to the needs of older adults living with ADRD and the needs of their caregivers. The primary objective of this funding opportunity is to proactively support IDoA’s Dementia Friendly Initiative as outlined in the FY 2022-2024 State Plan on Aging, FY2022-2024. The funding is dedicated to specifically accomplished strategy 2.6 K: Collaborate with partners and organizations with the capacity to develop an Illinois based education and technical assistance center for communities interested in becoming designated as a dementia friendly community. A secondary objective of this funding opportunity is to eliminate the disparities experienced by vulnerable and/or marginalized populations. Due in large part to the many social determinant of health and systemic factors that influence ADRD risks, research demonstrates that many vulnerable older adult population groups are impacted by ADRD at different rates, including: • Almost two-thirds (2/3) of individuals living with ADRD are women; • Older Black Americans are twice as likely to have ADRD than older White Americans; and • Older Hispanic adults are one and one-half times as likely to have ADRD than older White Americans.

Objective
The Dementia Friendly Illinois Communities Grant funds are to be used for the following activities: 1. Participate in monthly ICRN meetings and additional work group meetings throughout the project period. 2. Establish minimally one dementia friendly community in the PSA by 2024, if there is not a currently designated community. 3. Increase the number of dementia friendly communities by a minimum of one community by 2024 in the PSA to expand on existing dementia friendly communities already designated in the PSA.

The exact nature of the activities and the staff time required for the activities will vary based on identified community needs and priorities. AAAs may conduct the activities and educational opportunities through direct provision, partnership and/or contract with another organization, or utilization of a combination of the above. The Dementia Friendly Illinois Communities Grant funds are to be used for the following activities: 1. Participate in monthly ICRN meetings and additional work group meetings throughout the project period. 2. Establish minimally one dementia friendly community in the PSA by 2024, if there is not a currently designated community. 3. Increase the number of dementia friendly communities by a minimum of one community by 2024 in the PSA to expand on existing dementia friendly communities already designated in the PSA. • Grantees will complete either Activity #2 or Activity #3 dependent on the status of Dementia Friendly Community designations within their PSA. • Resources and instructions for tools, resources, and the four-phase implementation method can be found on the Dementia Friendly America website. Criteria Selecting Proposals
A review panel of at least three individuals will evaluate applications that pass the screening and meet the responsiveness criteria. These reviewers are experts in their field and are drawn from the current Illinois Department on Aging staff. Based on the application review criteria the reviewers will comment on and score the applications, focusing their comments and scoring decisions on the identified criteria. Final award decisions will be made based upon a point system. In making these decisions, the Office of Older American Services Supervisor will take into consideration recommendations of the review, of reviews from programmatic and grants management compliance, and the likelihood that the proposed project will result in the benefits expected. Successful applicants will receive a Notice of State Award (NOSA) signed and dated by the Grants Management Officer. A Uniform Grant Agreement will be sent to Grantee following the NOSA.

Audits
Non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503. In accordance with the provisions of Subpart F-Audit Requirements, under 45 CFR Part 75.500, nonfederal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year.
