These funds will be used as operational expenses by AgeOptions to: develop an outreach, education, and advocacy plan for underserved populations in the south suburbs, increase awareness of caregiver services across PSA 13, and enhance caregiver coordinator training across PSA 13.

Objective
• Develop a quarterly report with IDoA for measuring and monitoring of grantees Outreach, Education, and Advocacy in the South Suburbs: • Develop and submit a comprehensive outreach, education, and advocacy plan targeting underserved and marginalized population in the South Suburbs Caregiver Awareness Building Campaign: • Develop and submit a comprehensive multi-media campaign plan that reaches across the PSA in multiple modalities and languages. • Develop a mechanism to track the reach of this campaign and the response to media campaign to demonstrate which modality generated the most response. Enhanced Training for Caregiver Coordinators: • Develop and submit in person and virtual on-demand training modules for Caregiver Coordinators and other staff and volunteers who work with caregivers to enhance their knowledge surrounding the various programs, policies, and eligibility requirements for a wide range of potential services and supports to facilitate an increase in person centered planning that meets the needs each individual caregiver presents. • Develop and pretest and posttest to demonstrate efficacy of program.

In addition to the uniform requirements in PART ONE, the Grantor has the following additional requirements for its Grantee: 1. The Area Agency on Aging is required to submit Program Report of their Outreach, Education, and Advocacy Campaign progress and performance to IDoA on a quarterly basis. 2. The Area Agency on Aging is required to submit Program Report of their Enhanced Caregiver Training program progress and performance to IDoA on a quarterly basis. 3. The Area Agency on Aging is required to submit Program Report of their Caregiver Awareness Campaign progress and performance to IDoA on a quarterly basis. 4. The Area Agency on Aging is required to submit Monthly Financial Status Report. 5. The Area Agency on Aging is required to meet regularly with IDoA for collaboration on these deliverables and milestones.

Program Accomplishments
Outreach, Education, and Advocacy in the South Suburbs, $300,000: Grantee will cultivate deep community connections and increase visibility of the aging network resources to older persons and caregivers in the South Suburbs. Caregiver Awareness Building Campaign, $500,000: Grantee shall develop a caregiver awareness building campaign to expose all ages of the population to caregiver resources, connecting opportunities through Older American Services, and resources available through the aging network. AgeOptions will work collaboratively with IDoA on planning and messaging. Enhanced Training for Caregiver Coordinators $200,000 Grantee shall develop in-person and virtual training modules to train Caregiver Coordinators across their PSA.
