Short Description
Fox Valley Special Recreation Association (FVSRA) will be using funding to provide services to individuals with disabilities by the means of therapeutic recreational programs. At FVSRA, individuals experience new activities, make friends, and become part of a supportive community. With over 600 programs offered annually, individuals can stay active and healthy through fitness, athletics, creative arts, day programs, summer day camp, inclusion, and more. FVSRA has seen an increase in costs associated with effectively supporting individuals with disabilities. The medical and behavioral needs of the individuals is increasing requiring more, higher-skilled staff and additional training hours. The increase in minimum wage and other external laws, such as, Paid Leave for All Workers has significantly impacted the salaries line items of the Association. To ensure ADA compliance, FVSRA subsidizes a large portion of the program costs to be consistent with member park district/department programs. This ensures programs are affordable for those we serve. This grant will ensure the execution of these essential services to continue to meet the community needs by funding the staffing deficit of the Full-Time Therapeutic Recreation Specialists. These staff provide the program leadership and development of the therapeutic recreation programs provided. Ten of the 12 Therapeutic Recreation staff have their CTRS (Certified Therapeutic Recreation Specialist) certification. They design and lead most of the therapeutic recreation programs. FVSRA Therapeutic Recreation (TR) staff follow the APIED process. This includes participant and program Assessments, Program Planning, Implementation, Evaluation and Documentation of therapeutic recreation programs. Each program is intentionally designed with designated goals and objectives. Participants are encouraged to select programs based on their own individual goals as identified by their participant assessment.

Objective
The Therapeutic Recreation Program intends to: To design an individual recreation program, based on the individual’s program assessment data provided. To ensure that individual with disabilities can stay active and healthy through fitness, athletics, creative arts, day programs, summer day camp, inclusion, and more. To increase the number of participants enrolled in 2024 by 25% compared to the previous fiscal year. To have less than 10% of programs offered be cancelled due to low/no enrollment. To release over 50% of participants from the waiting lists and enroll them into the program.

Reports
A. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit financial reports to Grantor pursuant to Paragraph 10.1 and reports must be submitted no later than 30 days after the quarter ends. B. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 10.2 and no later than 30 days after this Agreement's end of the period of performance or termination. C. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit E, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 11.1 and such reports must be submitted no later than 30 days after the quarter ends. D. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 11.2 and no later than 30 days after this Agreement's end of the period of performance or termination. The Providers will submit reporting to the Division of Developmental Disabilities (DDD) on a quarterly basis, utilizing the Periodic Performance Report (GOMBGATU-4001). Quarterly reports will be submitted no later than 15 days after end of each report period. 1st Quarter Reports are due No Later Than (NLT) October 15th, 2nd Quarter Reports are due NLT January 15th, 3rd Quarter Reports are due NLT April 15th, 4th Quarter Reports are due NLT July 15th.
Audits
Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor's Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90
