Short Description
The Senior Farmers Market Nutrition Program provides vulnerable low-income adults age 60 and older $25 coupon booklets to purchase fresh fruits, fresh vegetables and honey at participating Farmers markets and roadside stands throughout the state.

Objective
These grant funds will assist in providing administration related funds to distribute SFMNP coupons and educate adults age 60 and older who qualify for the program how and where to use the coupons to purchase fresh fruits and vegetables and honey at local farmers markets.

Submits the required Title XX Social Services Block Grant (SSBG) data elements to IDoA by the assigned deadline using the required DHS/IDoA templates (i.e., 2025 Dual Participation List, 2025 SFMNP AAA Race Ethnicity Reporting Tool). IDoA will then submit statewide combined reports (i.e., 2025 Dual Participation List, 2025 SFMNP AAA Race Ethnicity Reporting Tool) for all participating AAAs and will provide support documentation of total SSBG expenditures for the reporting period (i.e., July 1, 2025 – September 30, 2025) to IDHS by the assigned deadline.

Award Procedures
Area Agencies on Aging will receive funds based on their redemption rates from the previous season. Program is from July 10, 2025, to October 31, 2025. DHS will transfer funds associated with this agreement to IDoA using fund 830. IDoA will subaward funds to participating Area Agencies on Aging (AAA) as a subgrantee for SFMNP. IDoA SFMNP provider awards will range from $2,500 to $5,000 per participating AAA with a base of $2,000. Additional funding on top of the base amount will be provided based on the SFMNP AAA’s 2024 season redemption rate, with higher amounts provided to SFMNP providers by tier: 69% redemption rate or lower will receive an additional $500; 70%-79% redemption rate will receive an additional $1,000; and 80% and above redemption rate will receive additional $1,500.

Formula Matching Requirements
Area Agencies on Aging will be awarded amounts based on their 2024 redemption rates with a base rate of $2000.
Uses and Restrictions
The awarded funds should be used to administer the Senior Farmers Market Program. This would include any costs associated with administering the program such as printing and providing handouts, office supplies and staff time. Restrictions: No equipment purchases.
Reports
The grantee or AAA shall submit to the Department the following reports using the required DHS/IDoA templates (i.e., 2025 Dual Participation List, 2025 SFMNP AAA Race Ethnicity Reporting Tool). IDoA will then submit statewide combined reports (i.e., 2025 Dual Participation List and 2025 SFMNP AAA Race Ethnicity Reporting Tool) for all participating AAAs and will provide support documentation of total SSBG expenditures for the reporting period (i.e., July 1, 2025 – September 30, 2025) to IDHS by the assigned deadline. Requires 1) Total Expenditures of Program including: SSBG Allocation + Expenditures of all other Federal, State and Local Funds (Non-SSBG funds) if applicable for SFMNP; 2) Total number of individuals, disaggregated by: Adults age 60-64 and Seniors (65 and above); 3) Race and Ethnicity Data: American Indian or Alaska Native; Asian; Black or African American; Hispanic or Latino; Native Hawaiian or Pacific Islander; White; Multiple Race; 4) Unique Identifier for each SFMNP participant ( unique client ID will be created by using the first SFMNP benefit check # issued to each SFMNP beneficiary as their unique identifier; 5) Type of eligible service provided: Health & Wellbeing – Health-Related Services for the season starting July 10, 2025-October 31, 2025.
