The Independent Service Coordination (ISC) Program provides education, referral, and linkage services for children and adults with developmental disabilities. Completion of Prioritization of Urgency of Need for Services (PUNS) database forms, as instructed by the Department, for all individuals in the assigned geographic area who seek inclusion in the database of unmet need. 2. Pre-Admission Screening. Principal services offered through the Pre-Admission Screening (PAS) program include the provision of prior authorization for all individuals for whom there is a reasonable basis to suspect the presence of a developmental disability who request Medicaid-funded services or nursing facility services. Specific PAS functions include the following: Conducting and securing assessments to determine service eligibility; Education of individuals, guardians, and families about service opportunities, including generic and specialized services; Notices of determinations provided to individuals and guardians; Assistance with the choice process; Referral; Assistance with service selection and linkage; Assistance in preparation and approval of submissions to the Department of Human Services (DHS) for capacity and rate requests when necessary (e.g., CILA and purchase of service programs); Monitoring for four weeks following initiation of services; and Accessibility 24 hours per day, 365 days per year for individuals in times of crisis. PAS activity is governed by the DDD PAS Manual, the Waiver Manual, and the Department of Healthcare and Family Services Universal Pre-Screening Rule (89 Ill. Adm. Code 140.642). PAS agencies must comply with these and related documents. The Department reserves the right to review and reverse all determinations by PAS agencies, in keeping with procedures and policies laid out in those documents and pertinent memoranda and policy statements issued by the Division and other governmental entities, such as the Department of Healthcare and Family Services. 3. Bogard - The Bogard Service Coordination program provides the following support services to Bogard Class Members: Eligibility Requirements: The individual must be an adult with a developmental disability who has been designated in writing by DHS as a Bogard class member. Bogard class members are defined in the modified Bogard Consent Decree. Performance Measures - Completion of all Deliverables shall be reported through Units of service per individual reporting via the Reporting of Community Services (ROCS) database. - Required Data Elements to be reported shall Include: Consumer Name; Demographic Information; Diagnosis; Assessment of the need for: 24 Hour Nursing Services and Active Treatment related to Developmental Disability. Performance Standards The Provider shall comply with Medicaid Waiver qualifications, as delineated in the federally approved Waiver. Should the Vendor be determined by the Department to be in non-compliance, the Vendor will enter into a Corrective Action Plan with the Department to remediate issues and achieve compliance. Formula Matching Requirements
The Department of Human Services requires a budget and determines grant funding by percentage of statewide population in service area. All of these Service Coordination programs utilize General Revenue (State) funds. There is no cost sharing or MOE.
Uses and Restrictions
There is no cost sharing or matching requirements for these programs. In order to charge indirect costs to this grant, the applicant organization must have an annually negotiated indirect cost rate agreement (NICRA). The applicant must submit a copy of the NICRA along with a budget.
Reports
REPORTING Reports are required on a monthly basis via the Reporting of Community Services (ROCS) database.Completion of all Deliverables detailed in Exhibit B shall be reported through Units of service per individual reporting via the Reporting of Community Services (ROCS) database. Required Data Elements to be reported shall Include: Consumer Name Demographic Information Diagnosis Assessment of the need for: 24 Hour Nursing Services Active Treatment related to Developmental Disability. AUDITS In accordance with the provisions of 2 CFR 200, Subpart F - Audit Requirements, non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503. RECORDS There is a 3-year records retention requirement; records shall be retained beyond the 3-year period if final audit has not been completed or findings resolved.
