This is a one-year non-competitive grant for $100,000.00 awarded to the Cook County Department of Public Health. Sexually transmitted infections (STIs) remain a significant public health problem in Illinois. In 2023, Illinois ranked31st for reported primary and secondary syphilis, 11th for chlamydia, and 13th for gonorrhea in the United States. Primary and secondary syphilis rates increased by 7% in 2023 compared to 2019. The majority of cases are among men (75%), specifically men who have sex with men (MSM), and trends of female rates continue to increase (128% increase in 2023 from 2019). As more females are being diagnosed with syphilis, the trends for congenital syphilis cases also continue to be high. In Illinois (excluding the city of Chicago), congenital syphilis cases are primarily found in Cook County. Cook County is the only county that has more than 1 congenital syphilis case reported each year for almost the past decade (since 2013). Suburban Cook County, on average, consist of37% (2018-2020) of all Illinois (excluding Chicago) reported congenital syphilis cases reported each year. The jurisdiction with the next largest average percent of congenital syphilis cases is St Clair County with an average of16%. The disease intervention specialists (DIS) at Cook County will conduct case investigations of female syphilis cases and congenital syphilis cases. Conduct counseling and partner services where needed. The services provided by this grant opportunity are essential to help prevent further transmission of syphilis, prevent congenital syphilis cases, and the resulting costly medical complications related to adverse conditions related to neuro-involvement due to syphilis (e.g., ocular manifestations, optic manifestations, and late manifestations) or congenital syphilis (e.g., stillbirths, birth deformities). The Illinois Department of Public Health shall fund the Cook County Department of Public Health to support syphilis prevention and containment activities in Cook County, excluding the city of Chicago, for one year (12months) starting January 1, 2026, through December 31, 2026. This grant opportunity shall help the Illinois Department of Public Health STI Section (ISS) accomplish the Centers for Disease Control and Prevention’s (CDC)Strengthening Sexually Transmitted Diseases Prevention and Control for Health Departments (STD PCHD) federalgrant objectives and activities.

Agency shall deliver the following in the Cook County Department of Public Health (CCDPH) jurisdiction, excluding the city of Chicago; 1. Provide syphilis prevention services predominantly among women with the highest priority towards pregnant women and women of reproductive age. 2. Provide counseling, partner services, risk assessment, sex partner referral, and culturally competent syphilis prevention. These services will be provided to individuals diagnosed with early syphilis and who are residents within the Cook County Department of Public Health’s jurisdiction. Services will be provided by a trained Disease Intervention Specialist (DIS) who has completed a Department approved STD counselor training. 3. Provide DIS counseling, partner services, risk assessment, and sex partner referral for at least 80% of female residents between the ages of 15 and 44 residing within CCDPH’s jurisdiction with syphilis. 4. Ensure females between 15 and 44 years of age residing within CCDPH jurisdiction have pregnancy status reported by at least 5% above baseline (2018 baseline for CCDPH: 24 cases out of 32 cases where pregnancy status was known and reported). 5. Provide assistance with linkage to healthcare/prevention care for at least 80% of females with syphilis residing within CCDPH’s jurisdiction who currently are without those resources. 6. Provide technical assistance to heath care providers and community organizations including but not limited to; initiating, reviewing and transmitting mandatory STD case reports to Cook County Department of Public Health. 7. Promote syphilis prevention and treatment services according to CDC guidelines. 8. Monitor and report cases of syphilis as required by law. 9. Provide an STD Monthly DIS Activity Report. 10. Provide ISS with Cook County Department of Public Health’s standard operating procedure document for the agency’s STD program by the end of first grant year. 11. Respond to syphilis incidence and prevalence trends and outbreaks as reported by ISS. 12. Develop and utilize targeted approaches that result in programmatic improvement, within their agency, that impact communities with high morbidity.
