Short Description
With this funding opportunity from IDHS, the DuPage Care Center will support the enhancement of its rehabilitative therapy program for long-term care residents and short-term rehabilitation.
This project focuses on increasing therapeutic service capacity through the staffing of key personnel and the procurement of essential rehabilitative equipment. The overarching objective is to improve physical outcomes mobility, and quality of life for DuPage Care Center resident population, many of whom experience chronic illness or complex medical conditions. The grant will fund salaries for several critical positions, including a Wellness Center Exercise Physiologist, two Physical Rehabilitation Certified Nurse Rehabilitation Aides (PR-CRNAs), and a licensed Physical Therapist. these staff members will work in DPCC's rehabilitation gym to deliver individualized therapy plans and facilitate group and one-on-one therapy sessions for residents. To complement these services, DPCC will procure an Omni Cycle Connect Active-Assist Cycle machine to replace an outdated and malfunctioning unit. This equipment will be used to provide low-impact, therapist-supervised exercise designed to improve strength, endurance and arrange of motion in residents undergoing rehabilitative care.

Objective

State agencies are federally required to evaluate the risk posed by each applicant. Risk assessments are a pre-award requirement and include two components: fiscal and administrative and programmatic. Illinois uses an Internal Control Questionnaire (ICQ) to complete the fiscal and administrative component and the risk assessment. Entities are required to access, complete and submit the ICQ through the Grantee Portal for the state fiscal year associated with the grant funding. Both the ICQ and the programmatic risk assessment must be accepted by the state and approved before a grant award can be executed.
Preapplication Coordination
Regardless of the source of funding (federal pass-through or State), all grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered.
