Short Description
CCBYS Placement Provider will provide successful and supportive placement options for CCBYS youth that may have had communication issues, disagreements, (1) who has (a) absent from home without consent of parent, guardian or custodian, or (b) beyond the control of his or her parent, guardian or custodian, in circumstances which constitute a substantial or immediate danger to the minor’s physical safety; and (2) who, after being taken into limited custody and offered interim crisis intervention services, refuses to return home after the minor and his or her parent, guardian or custodian cannot agree to an arrangement for an alternative voluntary residential placement or to the continuation of such placement. The youth that are referred to the Vendor may also have elevated issues like aggression and/or non-life threatening violence toward others, or (3) may threaten to hurt others, or (4) has a history of minor destruction of property, could have a pending non-violent charge or is out on parole, or (5) has a higher level of metal health needs of 70 IQ or above, may have an IEP, or (6) has a history of running, or (7) may have lower sexual aggression issues that are not criminal in nature.

Objective
CCBYS Placement Provider shall provide dedicated placement beds 24 hours a day 7 days a week to referring CCBYS agencies in need of a residential bed for a youth in their care. CCBYS Placement Provider will provide or arrange local transportation to and from the program location, as required by individual youth circumstance at no additional cost to IDHS. CCBYS Placement Provider will include when applicable cognitive behavioral therapy to reduce recidivism, along with elective courses including Personal Care/health and well-being, education, food management, money management, home management, housing, transportation, community resources and leisure time, and social skills.

Eligible Applicants
Public or private, not-for-profit community-based agencies that have a minimum of five (5) years’ experience providing services to at-risk youth. The funding opportunity is not limited to those who currently receive or previously received CCBYS grant funding.
Grantees must hold a current Child Welfare Agency license issued by the Department of Children and Family Services (See 89 IL Admin Code 401 Licensing Standards for Child Welfare Agencies.). A copy of your agency’s Child Welfare License or permit is to be included. The Grantee must have a valid Child Welfare Agency license by the due date of this application, or the application will be considered ineligible and will NOT be considered for funding
Beneficiary Eligibility
Crisis (Mandated)
Any minor, 11-17 years of age (1) who is (a) absent from home without consent of parent, guardian or custodian, or (b) beyond the control of his or her parent, guardian or custodian, in circumstances which constitute a substantial or immediate danger to the minor's physical safety; and (2) who, after being taken into limited custody and offered interim crisis intervention services, refuses to return home after the minor and his or her parent, guardian or custodian cannot agree to an arrangement for an alternative voluntary residential placement or to the continuation of such placement.
Lock-outs: any minor, 11-17 years of age whose parent or caregiver has denied the child access to the home and has refused or failed to make provisions for another living arrangement for the child, will be considered a lockout.
Non-Crisis (Discretionary)
Any youth who are 11-17 years of age and at risk of involvement with the child welfare and/or juvenile justice system may be eligible for CCBYS non-crisis services. If the youth does not meet the criteria for the Crisis Population above and is assessed and considered at risk of crisis or at risk of delinquency, or both, the youth would be eligible for services under this section.

Preapplication Coordination
a) Pre-application Coordination.
Applicant agencies may not be eligible for a grant award until they have pre-qualified through the Grant Accountability and Transparency Act (GATA) Grantee Portal, www.grants.illinois.gov. During pre-qualification, Dun and Bradstreet verifications are performed including a check of Debarred and Suspended status and good standing with the Secretary of State. The pre-qualification process also includes a financial and administrative risk assessment utilizing an Internal Controls Questionnaire. If applicable, the agency will be notified that it is ineligible for award as a result of the Dun and Bradstreet verification. The entity will be informed of corrective action needed to become eligible for a grant award.
b) The Application Procedure.
An Application must be submitted in the format required by the Department and in the manner dictated by the Department.

Uses and Restrictions
Provider will provide and operate a residential placement option with cognitive behavioral services and structured recreational and learning activities to reduce risk of recidivism and promote positive youth development:
1. The Provider shall operate a residential placement option for no less than Bed # CCBYS youth at a time if referred by the CCBYS home agencies; and youth will participate for a period of approximately no more than (21) business days. Any extensions for youth participation past (21) days must be pre-approved by the CCBYS program administrator and the Provider.
2. Hours of operation shall be (24) hours a day (7) a week with an ability for afterhours intake options.
3. The Provider shall provide placement services through its Illinois residential facility and may include the following when applicable:
a. Individualized case management.
b. Cognitive Behavioral Therapy.
c. Educational and vocational training programs (when applicable).
d. Physical fitness and recreational activities.
e. Jobs training and long-term placement (when applicable).
f. Jobs training and long-term placement (when applicable).
4.The Provider shall provide a daily schedule of activities focused on structure and youth’s needs. Schedule shall be made available to youths, CCBYS home agency staff and IDHS when requested.
5. The Provider shall provide transportation for youth to attend the local programing, local schooling, and local doctors’ visits.
6. The CCBYS home agency will provide the Vendor the Youth Assessment Screening Instruction (YASI) at the point of referral (if available) and when re-assessments are completed. The YASI will be the basis for the Vendor to develop an individualized transition plan for each youth.
7. The Provider shall provide linkages to community-based services for youths based on the YASI (if applicable).
8. The Provider shall provide services in the primary language or method of communication of each youth and, as applicable, to the youth’s parent(s), guardian(s), and/or significant persons involved with the youth or work with IDHS/CCBYS to provide that service.
9. The Provider shall provide weekly updates on each youth’s status to the home CCBYS Agency. When a youth has been discharged from the vendor the transition plan shall be shared with the youth and CCBYS Agency.
10. The Provider shall provide advisors whose primary function is to support the youth in learning life skills and developing positive relationships.
Reports
A.) Reports.
1. The Provider will submit monthly expenditure documentation forms in the format prescribed by the Department. The Expenditure Documentation forms must be submitted no later than the 15th of each month for the preceding month by email.
2. The Provider will submit an annual program plan and a detailed program budget and budget narrative to the Department as directed.
3. Monthly and quarterly data reports must be submitted no later than the 15th of each month for the preceding month by email.
4. Additional annual performance data may be collected as directed by the department and in a format prescribed by the Department.
B) Audits.
Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and subpart F of 2 CFR Part 200, and the audit rules set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65©.
C) Records.
