Short Description
The Illinois Breast and Cervical Cancer Program (IBCCP) is a Statewide Cooperative Grant Agreement Program funded by the Centers for Disease Control and Prevention (CDC) and State General Revenue Funds. IBCCP offers free mammograms, breast exams, pelvic exams, Pap tests, screening, and diagnostic procedures to eligible women in Illinois. Authorized through the Breast and Cervical Cancer Mortality Prevention Act of 1990 (Public Law 101-354), the program ensures that eligible Illinois women receive regular screening for breast and cervical cancer, prompt follow-up of abnormal findings, when needed, and to ensure that all screening and diagnostic tests are performed according to current recommendations for quality assurance.

Objective
To reduce breast cancer and cervical cancer deaths. • To provide breast and cervical cancer screening services to eligible Illinois women, with an emphasis on women of low income, racial/ethnic minorities, rarely or never screened and older women. • To increase access and utilization of early detection measures. • To reduce barriers that keep Illinois women from seeking services. • To provide appropriate referrals, when needed. • To ensure appropriate and timely follow-up, diagnosis, and treatment through case management. • To develop and disseminate culturally sensitive public information and education programs. • To determine the education needs of health care professionals relating to breast and cervical cancer screening and to offer educational opportunities that increase the skills and knowledge of those professionals. • To monitor the quality and interpretation of screening/diagnostic procedures. • To monitor trends in breast and cervical cancer incidence, mortality, and screening rates for Illinois women. • To involve the IBCCP Coalition and the IBCCP Medical Advisors in the development and guidance of the Program. • To evaluate these activities through surveillance systems. • To ensure a risk assessment is completed on all IBCCP clients.
The Illinois Breast and Cervical Cancer Program (IBCCP) is issuing two non-competitive grants to cover the southern Illinois counties: Edwards, Franklin, Gallatin, Hamilton, Jackson, Jefferson, Monroe, Perry, Randolph, Saline, Wabash, Washington, Wayne, White, and Williamson. IBCCP is a breast and cervical cancer screening program funded by the Centers for Disease Control and Prevention’s (CDC) National Breast and Cervical Cancer Early Detection (NBCCEDP) and the State of Illinois general revenue funds. The program provides free breast and cervical cancer screening and diagnostic testing to eligible clients in 102 counties through 28 Lead Agency sites in Illinois. The application will be available through EGrAMS between February 17, 2025, through March 17, 2025. The initial contract term will be from January 1, 2025, through December 31, 2025. The funding amount by budget category is provided and is non-negotiable. Funding is based on a per person (caseload) formula and is meant to supplement the grantee’s current work and funding. The Grantees awarded the contracts will be responsible for attaining the assigned caseload for the coverage area. Caseload increases will not be allowed until after the first year.

Preapplication Coordination
all grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, https://gata.illinois.gov/, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI (Unique Identity ID) number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
The application format is provided via e-GRAMS as mandated for all Illinois Department of Public Health Grants. While some of the sections in this application relate specifically to IBCCP, many sections must be completed for all grant programs. The Application must be completed in its entirety and submitted by May 20, 2022. After you have completed your application in e-GRAMS, make sure to print a copy and click the “Submit” button. Applications will require 14 to 21 business days for review and approval. Official notification, which allows for expenditure of funds, will be the receipt of a fully executed grant agreement.

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.

Renewal allowed if both parties agree.
Uses and Restrictions
Funds must be used to screen women, provide referrals; ensure follow-up on abnormal screenings, develop and disseminate educational materials; training, and skills of health professionals; monitor the quality and interpretation of screening procedures; and, evaluate these activities through surveillance systems.
Reports
The following reporting is required: Grantees will submit a Project Status Report each quarter of the Fiscal Year that illustrates the grantees success in meeting the specific measurable goals, objectives, and tasks outlined in the work plans. In addition, complete the Quarterly Outreach and In-reach Activities Report included with the work plans. The Mid-Year Desk Audit will be due on or about December 31st. Specific information regarding content and due date of the Mid-Year Desk Audit will be provided during November. Grantees who receive $150,000 or more in funding will provide a single audit report within one hundred-eighty (180) days after the Grantee's fiscal year ending on or after June 30th of each fiscal year. Cornerstone HSPR0785 - BCCP Open Screening Cycle Follow-Up Report, which is to be run monthly using the last working day of each month as the time frame. Cornerstone HSPR0897 - BCCP Summary Statistics Report, which is to be run on a monthly basis, using dates to show cumulative information from the first day of the current Fiscal Year (July 1) to the end of the current month. Cornerstone HSPR0783 - BCCP Detailed Procedure and Reimbursement Report (clinical/Navigation Only/RTTA), which is to be run on a monthly basis on the 15th day of the following month. Reimbursement Certification Form for CSS/CBN/CM/Navigation Only, which is to be run on a monthly basis on the 30th day of the following month. Grantees will submit Consolidated Financial Reports within sixty (60) days after the Grantee's fiscal year ends on or after June 30. Grantees will report to IBCCP staff and address changes as they occur. Grantees will submit two updated Medical Provider List reports, one with application and another with the Mid-year desk audit. Grantees are also required to report to IBCCP changes to the Medical Providers as they occur.
Audits
The Program requires an onsite audit every 3 years and a mid-year desk audit annually. Grantees who receive $150,000 or more in funding will provide a single audit report within one hundred-eighty (180) days after the Grantee's fiscal year ending on or after June 30th of each fiscal year.
Records
Agencies must retain client records for 6 years and financial records for 3 years.

Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to: i. The State awarding agency may not make a Federal pass-through or State award to an applicant until the applicant has complied with all applicable UEI and SAM requirements and, if an applicant has not fully complied with the requirements by the time the State awarding agency is ready to make a Federal pass-through or State award, the State awarding agency may determine that the applicant is not qualified to receive a Federal pass-through or State award and use that determination as a basis for making a Federal pass-through or State award to another applicant.
Obligations
Grantees may use funds only for reasonable program purposes, including personnel, fringe, travel, supplies, contractual services, rent/utilities, administrative cost, clinical services, training and education.

Program Accomplishments
To date, the IBCCP has provided approximately 724,344 high quality screening services to 232,909 clients and has referred 7,375 clients to treatment in a timely manner, which is demonstrated by the IBCCP’s ability to successfully meet its Core Indicators and MDE requirements annually.
