Objective
Illinois State Government and the U.S. Centers for Disease Control and Prevention (CDC) allocate grant funds to the Illinois Department of Public Health (IDPH) to provide HIV prevention services to Illinois residents at highest risk for transmitting or acquiring HIV Disease. This NOFO will select one Lead Agency for each of the eight (8) HIV Prevention Regions (see Table 1 and map below) to implement the service objectives needed from each region to achieve the Department’s PS18-1802 objectives. These objectives are compiled by region, by service type in the attached SFY23 Regional Service Plan. The initial grant will fund Year 1 of a possible three-year grant cycle, July 1, 2022, through June 30, 2023. This grant opportunity includes an option of two one-year renewals, subject to a successful performance evaluation, not to exceed a total of three years. Priorities for the highest risk populations to be served and the interventions to be provided to them are established annually by the Department, informed by recommendations of the Illinois HIV Integrated Planning Council (IHIPC). Prioritized risk populations and interventions are included in the most current Illinois Prioritized Risk Group Definitions and Points of Consideration and the most current HIV Prevention Intervention & Strategy Guidance for IDPH HIV Prevention Grantees respectively.

Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the GATA section of the IDPH website (www.dph.illinois.gov/GATA). o Appeals must be received within 14 calendar days after the date that the grant award notice was published. o Appeals must include the following information: ? The name and address of the appealing party ? Identification of the grant ? A statement of reasons for the appeal ? If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. o The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. o The appealing party must supply any additional information requested by the agency within the time period set in the request. o The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required. Documentation of the appeal determination shall be sent to the appealing party and must include the following: o Standard description of the appeal review process and criteria Page 13 of 14 (Updated 4/2019) o Review of the appeal o Appeal determination o Rationale for the determination In addition to providing the written determination, the grant-making office may do the following: o Document improvements to the evaluation process given the findings and re-review all submitted applications. o Document improvements to the evaluation process given the findings and implement improvements into the following year's grant evaluation process. o Provide written notice to the appealing party as to how the identified actions will be remedied. Appeals resolutions may be deferred pending a judicial or administrative determination when actions concerning the appeal have commenced in a court of administrative body.

Program Accomplishments
Organize service delivery to fill identified gaps in existing regional services so combined regional services tracked in Provide Enterprise will mirror recent regional HIV incidence proportionally by risk by race/ethnicity. • Select local HIV prevention service providers in the respective region with expertise in HIV prevention service delivery; execute local sub-grants with objectives that collectively implement the Regional Service Plan, filling service gaps identified by the Department in overall regional HIV prevention services relative to recent HIV incidence; and document those sub-grant awards and service objectives in Provide Enterprise. • Convene monthly Regional Implementation Group (RIG) meetings with regional service providers to deliver policy and procedure updates; and monitor and coordinate services in accordance with the contract. This monthly meeting must cover actual service delivery and health outcomes based on service objectives and performance standards specified in the scope of work. • Provide subgrantees with monitoring, technical assistance, and training to ensure compliance with service provider training requirements, achievement of service objectives, attainment of performance standards, fidelity to effective interventions and public health strategies’ protocols, and compliance with IDPH service and documentation guidelines; document the Department-required trainings of all regional subgrantee staff in the counselor set ups within Provide Enterprise by December 31, 2022. • Conduct ongoing quality assurance of subgrantee HIV prevention programs and Routine HIV testing sites as outlined in the Section's quality assurance guidance, including biannual site visits and intervention observations, and provide action plans to facilitate service objective completion, improve quality and correct any deficiencies. • Submit to the Department, along with grant application, a detailed monitoring and evaluation plan including SMART process and outcome objectives demonstrating how established Routine HIV Testing sites will be supported, monitored, and evaluated. • Ensure that subgrantees document risk-based HIV testing, HIV risk reduction activities, Surveillance- Based Services, routine HIV screening and supplemental services data in Provide Enterprise by required reporting deadlines. • Ensure that a minimum of 70% of services delivered by subgrantees fulfill scopes of services assigned to the Lead Agency by the Regional Service Plan. • Oversee and document condom distribution activities in sites established as high risk for acquiring and transmitting HIV. • Empower increased engagement of Black and Latino Men who have Sex with Men (MSM) into Prevention services by implementing social networking strategy, social marketing using CDC-developed materials, social media using CDC-developed digital materials, and peer outreach staff hiring and training. • Assist with any HIV Section-requested focus group activity, regional prevention resource assessments, and Regional Integrated HIV Engagement Meetings. • Build regional capacities to collect venous blood samples for HIV and Syphilis testing. • Regularly update a referral list of regional licensed health care professionals able and willing to prescribe for biomedical interventions such as Pre-exposure Prophylaxis (PrEP) and non-occupational Post-exposure Prophylaxis (nPEP). • Conduct program monitoring, evaluation, and quality assurance for all Surveillance-Based Services- related requirements for its interventions, strategies, training, and documentation according to the current Department SBS guidance. Page 4 of 14 • Collaborate closely with the regional HIV treatment providers, including the Ryan White Lead Agency, to ensure every willing testing- or surveillance-identified HIV-positive individual is referred to and engaged in HIV treatment within 30 days of receiving a preliminary positive result or being referred to Surveillance-Based Services. • Conduct monthly Region
