Short Description
Designed and operated by individuals with disabilities, Centers for Independent Living (CILs) provide independent living services for people with disabilities for people with all types of disabilities. CILs are at the core of ACL's independent living programs, which work to support community living and independence for people with disabilities across the nation based on the belief that all people can live with dignity, make their own choices, and participate fully in society. These programs provide tools, resources, and supports for integrating people with disabilities fully into their communities to promote equal opportunities, self-determination, and respect.

Objective
Centers for Independent Living (CIL) are private, not-for-profit, community-based organizations that provide programs and services for people with disabilities that allow them to live as independently as possible within their communities located throughout the state of Illinois. The Grantee Center will utilize the federal and State funds provided by this contract to provide those services required of Centers under: The Rehabilitation Act of 1973; 34 CFR 364-366; 45 CFR 1329; and the Illinois State Plan for Independent Living. These services include at a minimum: 1. Information and Referral (I&R) - Information and referral is the art, science and practice of bringing people and services together. If individuals or families don't know where to turn, or need more information, I&R can help connect them with the information, tools, and resources they need 2. Independent Living Skills Training - CILs may provide information and instruction which lead to mastery in the following areas: health and safety, community resources, meal planning and nutrition, self-advocacy, basic legal concepts, Personal Assistant management, household management, recreation and leisure, and money management. The integration and full participation of individuals with disabilities into the mainstream of society, and the reduction of institutionalization, is the desired outcome. 3. Peer Counseling - CILs provide support and role models for people who want to live more independently. Personal experience with disability often places CIL staff in a unique position to understand and effectively teach people who experience disabilities while providing a role model for success. 4. Individual and Systems Advocacy - The goal is to empower people of all ages with disabilities to reach their highest level of achievement by teaching them to advocate for individual and systems changes that result in access, choice, equality, inclusion, integration and independence. Systemic advocacy attempts to change laws and attitudes surrounding disability. Personal advocacy is a type of problem-solving designed to protect personal and legal rights and ensure a dignified existence. 5. Transition Services - CILs provide services that facilitate transition from nursing homes and other institutions to the community, provide assistance to those at risk of entering institutions, and facilitate transition of youth to postsecondary life. 6. 6. Working with the Statewide Independent Living Council to develop and implement the State Plan for Independent Living.

Independent living programs are supported through funding authorized by the Rehabilitation Act of 1973, as amended (The Act). To receive funding, states must jointly develop and submit a State Plan for Independent Living (SPIL), which is a three-year plan for providing independent living services in the state. The Designated State Entity (DSE) is the agency that, on behalf of the state, receives, accounts for and disburses funds received under Subpart B of the Act. The Statewide Independent Living Council (SILC) is an independent entity responsible to monitor, review, and evaluate the implementation of the SPIL. Centers for Independent Living (CILs) are consumer-controlled, community-based, cross-disability, nonresidential private non-profit agency that are designed and operated within a local community by individuals with disabilities, and provides an array of independent living services.

All entities are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) Grantee Portal, www.grants.illinois.gov/portal to be eligible apply for a grant, How To Access The Grantee Portal. An Illinois.gov public account authenticates the relationship between the individual and the organization they represent, How to create an Illinois.gov Public Account. The following information is required to complete registration: 1.Organization's Unique Entity Identifier (UEI); 2.Organization's federal employer identification number (FEIN); 3.Organization type; 4.Illinois Secretary of State File ID (required for non-profits, for-profits and limited liability corporations); 5.Organization's name 6.Organization's mailing address; 7.Organization's primary email address; 8.Organization's primary phone number 9.Organization's fiscal year-end date Entities are federally required to have an active SAM.gov entity registration, How to register with SAM.gov. Illinois uses SAM.gov automation to determine qualification status based on the entity's registration. Refer to GATA Administrative Rules on Grantee Pre-qualification Status: http://www.ilga.gov/commission/jcar/admincode/044/044070000A00700R.html. An entity may apply for a grant award prior to qualified status. The entity must be in "qualified status" based on the Grantee Portal by the Application Review Date. See #18 in the NOFO Transmittal. [Non-competitive applications aren't subject to MBR so the more generic term Application Review Date vs. Merit Based Review Date may be appropriate.] The Grantee Portal will indicate in red if a status is not qualified. State agencies are federally required to evaluate the risk posed by each applicant. Risk assessments are a pre-award requirement and include two components: fiscal and administrative and programmatic. Illinois uses an Internal Control Questionnaire (ICQ) to complete the fiscal and administrative component and the risk assessment. Entities are required to access, complete and submit the ICQ through the Grantee Portal for the state fiscal year associated with the grant funding. Both the ICQ and the programmatic risk assessment must be accepted by the state and approved before a grant award can be executed.
Preapplication Coordination
Centers for Independent Living will not be eligible for a grant award until they have pre-qualified through the Grant Accountability and Transparency Act (GATA) Grantee Portal, www.grants.illinois.gov Grantee Links tab. Registration and pre-qualification are required annually. During pre-qualification, verifications are performed including a check of federal Debarred and Suspended status on the Illinois Stop Payment List and good standing with the Secretary of State. An automated email notification is sent to the entity alerting them of “qualified” status or providing information about how to remediate a negative verification. A federal Debarred and Suspended status cannot be remediated. Applicants must be pre-qualified, therefore, applications from entities that have not completed the GATA pre-qualification process prior to the due date of this application will NOT be reviewed and will NOT be considered for funding. A statement indicating the applicant has completed Pre-Qualification steps and is currently Pre-Qualified will be required with the application. The Provider’s proposed budget must be entered into the CSA system. The completed budget must be electronically signed and submitted in the CSA system, and a printed copy of the signed and submitted budget must be included with the application. To do this, the following is required: at a minimum, the applicant agency’s Chief Executive Officer (CEO) or equivalent, or the Chief Financial Officer (CFO) or equivalent must be registered in the CSA system to electronically sign the required budget documents prior to submission. Budgets not submitted as described here and by the due date and time will not be considered. 3. Unique Entity Identifier(UEI) Number and System for Award Management (SAM) Each applicant is required to: a) Be registered in SAM before submitting the application. The following link provides a connection for SAM registration: https://www.sam.gov/SAM/; b) provide a valid UEI number in its application; and c) continue to maintain an active SAM registration with current information at all times in which the applicant has an active Federal, Federal pass-through or State award or an application or plan under consideration by a Federal or State awarding agency. DHS may not make a Federal pass-through or State award to an applicant until the applicant has complied with all applicable UEI and SAM requirements and, if an applicant has not fully complied with the requirements by the time DHS is ready to make the award, DHS may determine that the applicant is not qualified to receive the award and use that determination as a basis for making the award to another applicant. 4. Unique entity identifier and System for Award Management (SAM) Each applicant is required to: (i) Be registered in SAM before submitting its application; (ii) provide a valid unique entity identifier in its application; and (iii) continue to maintain an active SAM registration with current information at all times during which it has an active award or an application or plan under consideration by the Department. The Department may not make an award to an applicant until the applicant has complied with all applicable unique entity identifier and SAM requirements and, if an applicant has not fully complied with the requirements by the time the Department is ready to make an award, the Department may determine that the applicant is not qualified to receive an award and use that determination as a basis for making an award to another applicant. Please refer to Section C. Eligibility Information & Grant Funding Requirements for additional information and detail regarding SAM. The pre-award process includes a financial and administrative risk assessment utilizing an Internal Controls Questionnaire (ICQ). The Department may NOT issue a Notice of Award or a Grant Agreement to any applicant that does not have a submitted and approved ICQ. While these are NOT required prior to submitting the application, they are required prior to the Department issuing an award. These grantee pre-award requirements are mandated by Federal Uniform Guidance (2 CFR 200) and the Grant Accountability and Transparency Act (GATA). Grantees must complete these requirements prior to receiving a grant award from the State of Illinois.

Formula Matching Requirements
Matching Requirements: Matching Requirements: Under section 712(b) of the Rehabilitation Act, the State must contribute $1 for each $9 of Federal grant funding received, in cash or in kind. This program does not have MOE requirements. All awards issued will contain both federal funding and state match. The final award allocation between federal and state funds will be based upon the Division's operating needs.
Uses and Restrictions
The State must submit a State Plan for Independent Living (SPIL) that meets requirements in section 704 of the Rehabilitation Act and 34 C.F.R. Part 364 of the Independent Living program regulations. The 3-year State plan must be submitted not later than July 1, of the fiscal year preceding the first fiscal year of the forth coming 3-year period. Federal laws and regulations prohibit federal grantees, which includes recipients of funding related to the Rehab Act as Amended, from using federal funds to lobby government officials (18 U.S.C. § 1913; 31 U.S.C. § 1352; 2 C.F.R § 200.450).

Monthly: Financial Report - A Monthly Grant Invoice (MGI) is required initiate payment Quarterly: Periodic Financial Report / Periodic Performance Report Annual Reporting Requirements: A. The ACL CIL Program Performance Report (ACL CIL PPR) is required by both the DHS DRS Independent Living Unit (ILU) as well as the US Department of Health and Human Services' Administration on Community Living (ACL). Those Grantees who receive Title VII Part C funding and are therefore allowed to submit the ACL CIL PPR directly to ACL, must submit copies of the report to the ILU by December 31st following the end of the federal fiscal year being reported. Those Grantees who do not receive Title VII Part C funding are required to complete the ACL CIL PPR, and then subsequently send the report to the ILU by November 15th following the end of the federal fiscal year being reported. Reports from the non-Part C funded Grantees will be used by the ILU and SILC to compile a combined report which the ILU and/or SILC will then submit to ACL directly. B. A Final Reconciliation is the final financial closeout of the award.
