Short Description
The provider will provide Pregnant Teens Doula services at 3-4 locations throughout the state. The goal of services is to provide support to pregnant teens and young adults at a critical time in their lives. The three to four programs work in partnership with medical providers in local hospitals. Clinical consultants for this group of sites include a Registered Nurse, a Family Nurse Practitioner, a Certified Nurse Midwife and an Advance Practice Nurse, among other credentials.

Objective
Deliverables: The Provider will work toward meeting the following: 1.Provision of Services a) Provide Doula services to a minimum of 88 unduplicated participants. b) Address offering support for both the teenage mother and the infant during pregnancy, delivery, and the weeks after the birth. c) Counsel pregnant women and their families on the birthing process, breast-feeding, developing the mother-child relationship, and child infant development. d) Provider will provide Doula services to participants at a minimum of three to four sites. e) Participants will all be low-income teen mothers, the majority of whom are Temporary Assistance for Needy Families (TANF) eligible. f) Doula services will be offered during the prenatal and perinatal period, and for up to 12 weeks postpartum for 22-51 participants at each site and will include the following: i) Prenatal classes. ii) Developing birthing support plans. iii) Frequent prenatal home visits. iv) Accompanying pregnant teens through labor and delivery. v) Extensive postpartum home visiting. g) Doulas will work in conjunction with home visiting staff at each site, transitioning teen parents into ongoing home visiting and a support group once the postpartum period has elapsed to achieve the following outcomes including: i) Increased rate of initiation of breast-feeding. ii) Collect and report data on rate of cesarean section births. h) Participants at the sites referenced in Requirement D will be screened for postpartum depression. i) Doula prenatal care and postnatal care to women shall assess new mothers for postpartum mood disorder symptoms at a prenatal check-up visit in the third trimester of pregnancy and at the initial postnatal check-up visit thereafter until the infant's first birthday. j) The provision of prenatal and postnatal care to a woman shall include fathers and other family members, as appropriate, in both the education and treatment processes to help them better understand the nature and causes of postpartum mood disorders. k) Will provide perinatal depression educational materials to include but not be limited to at a minimum the DHS Perinatal Depression Brochure which can be ordered from DHS at no charge to the Provider. l) The Provider will provide intensive training and technical assistance to these program sites, using the Provider's staff and/or consultants from the Chicago Health Connection (CHC) or other qualified resources. Clinical supervision will also be provided for Doulas at the two sites where there are no medical personnel.

Application Procedures

Registration and Pre-Qualification: Prior to applying for any Notice of Funding Opportunity (NOFO), every applicant must first be registered and prequalified through the following steps. This must be done on or before the application's due date or their application CANNOT be accepted. •Apply for or update their DUNS number (https://www.dandb.com/ ). This must be done yearly. •Apply for or update their SAM registration and receive a SAM cage code. (https://www.SAM.gov) This must be done yearly. •Be registered and in good standing with the Illinois Secretary of State. (http://www.cyberdriveillinois.com/ ) (This is not required of governmental entities and schools.) •Register with the GATA/CSFA system at https://grants.illinois.gov/registration/ . The system will automatically check your registrations with the above mentioned systems. It will also check to see if your entity is on the federal excluded parties list and if your entity is on the State of Illinois Stop Pay/Compliance list. If your entity is not up-to-date on the registrations, or is listed on the federal list or Illinois Stop Pay/Compliance list, your application will not be accepted until these situations are resolved. If a change to the registration information is needed at a later date, grantees may re-enter the system at https://grants.illinois.gov/registration/login.aspx . No applications can be accepted for review until these steps are successfully accomplished. Grant Application, Program Plan (if applicable), Budget, and Programmatic Risk Assessment: At the time of application, the grantee must submit the grant application, the program plan (if applicable), the budget, and the programmatic risk assessment. Fiscal and Administrative Risk Assessment: Grantees must complete the Fiscal and Administrative Risk Assessment on the GATA/CSFA system- also known as the ICQ (short for Internal Controls Questionnaire). Be sure to click "submit" to submit your answers when complete. This is done only once per entity per fiscal year. While it does not have to be completed prior to submitting the application, this step must be done before an applicant or their application can be considered. Indirect Cost Rate: Most, but not all grants will include the potential for a grantee to claim an Indirect Cost Rate to help cover the administration costs related to the grant. For new grantees or new grants, this must be done within 90 days of the grant's effective date. For returning grantees, this must be done within six months following the end of the grantee's fiscal year. All grantees must make a selection. Grant Selection, NOSA and Grant Agreement: If your application is selected for funding, the results of the ICQ and Programmatic Risk Assessment may render conditions that will be included in your Notice of State Award (NOSA). The NOSA will be generated from GATA's CSFA system, and may be delivered via email from that system. (if you change staff or email addresses, be sure to update your GATA registration with that information or the correct person or email will not receive the NOSA.) These conditions may need to be addressed prior to the award becoming a firm grant agreement or they may be addressed over the course of the timeframe of the award. The proposed IDHS grantee must sign-off and return the NOSA to IDHS. Once the grantee accepts the IDHS grant and returns it, IDHS will also accept and the contracting process begins.

Quarterly Reporting using Periodic Performance Reports (PPRs) and Periodic Financial Reports (PFRs). Supplemental reporting, as outlined in Exhibit B of the grant agreement, is also required.

Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless a different retention period is specified in 2 CFR 200.334 or 44 Ill. Admin. Code 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records must be retained until all litigation, claims or audit exceptions involving the records have been resolved and final action taken.
