Short Description
This competitive grant application will provide funding to agencies that are interested in providing direct targeted HIV and HCV testing services to individuals who are at high risk for acquiring HIV and HCV. Funds will support initial screening and confirmation for populations at greatest risk of acquiring HCV, mainly People who inject drugs (PWID) and for targeting screening for HIV according to the 2018 Prioritized Populations representative of the most recent surveillance and epidemiologic profiles of populations most at-risk. Grant funds will include Fee-for-service testing, supplemental services, capacity building, and indirect costs funds to assure screened and confirmed HIV/HCV cases receive necessary linkage to care and medical treatment, insurance application assistance, referrals to services needed to protect health including prevention interventions, and substance abuse treatment and provision of safer injection materials including sterile syringes and naloxone through comprehensive harm reduction services.

Objective
The goal of these grants are to diagnose and treatment-engage Illinois residents living with HIV and HCV who are unaware of their status.

Applicant Eligibility
An organization may apply if they have prior experience conducting the following activities listed below in a grant from the Illinois Department of Public Health HIV or STD Prevention programs, or the Center for Minority Health Services or Local Health Protection Grant Programs, the Illinois Division of Alcohol and Substance Abuse or the Chicago Department of Public Health. The applicant must demonstrate how they have effectively engaged prioritized risk populations for HIV including gay or bisexually active males, high risk heterosexual (HRH) women and men, persons who currently or formerly injected drugs (PWID), particularly those who are transgender or who are representative of racial or ethnic minorities impacted by poverty.

Application Procedures
Applications must be submitted and certified in EGrAMS.
Criteria Selecting Proposals
An objective and merit-based grant review and evaluation process shall be conducted in EGrAMS to make the final program funding determinations for the year 1 (July 2021-June, 2022) grant awards. This application shall be scored in the following categories: Past Grant Service Delivery Performance, Agency Background and Historical Experience, Project Description Narrative, Overall Project Goals, Objectives and Timelines, Project Evaluation and Assessment Plan, Health Equity, and Detailed Budget and Justification. A score of 75 or above may be considered eligible for funding.

Appeals may be submitted in writing to the HIV Section Chief.
Renewals
IDPH may optionally renew SFY22 grantees with satisfactory performance for an additional two years.

Funding may be used to implement the defined scope of testing services, supplementary services, capacity-building activities, and indirect costs.
Reports
Quarterly reports are due 15 calendar days after the close of the quarter. An End-of-Year report is due 15 days after the close of the project period.
