Short Description
Grantees are designated delegate agencies of the Illinois Department of Public Health for the purpose of carrying out the provisions of the Lead Poisoning Prevention Act (410 ILCS 45/1) and Lead Poisoning Prevention Code (77 IAC, Part 845). Lead Poisoning, if not addressed, may lead to substantial developmental issues, learning difficulties, behavioral problems, and possibly death. Grantees will, at a minimum, conduct case management home visits and environmental investigations at regulated facilities where a child who has an elevated blood lead level resides or frequents.

Lead Poisoning Prevention Act (410 ILCS 45/1)
Objective
Local Health Department (LHD) will provide case management and environmental investigation services for children identified with a confirmed blood lead level of 10 micrograms per deciliter or greater. LHD may also provide services to children identified with a confirmed blood lead level of 5-9 micrograms per deciliter when the doctor requests intervention. In conjunction with the case management and environmental investigations, the LHD will participate in public awareness and education campaigns regarding lead poisoning prevention.

Grant is only available to certified local health departments in Illinois that choose to conduct lead poisoning prevention activities. The grant provides funding to conduct home visits and (if able) environmental investigations for children with a confirmed elevated blood lead level in their jurisdiction.

Formula Matching Requirements
Surveillance data has been used to determine the number of lead poisoned children (=10µg/dL) in each jurisdiction. Each lead poisoned child is assumed to receive a case management home visit with a reimbursement rate at $75 per child. Additionally, we’ve incorporated base awards for each delegate by utilizing surveillance data to determine childhood blood lead testing rates in each jurisdiction. The statewide average childhood blood lead testing rate is 23% and is being used to develop four separate tiers for the base awards: 1. =15% testing rate will receive a base award of $2,500 2. 16 – 23% testing rate will receive a base award of $5,000 3. 24 – 30% testing rate will receive a base award of $7,500 4. =31% testing rate will receive a base award of $10,000 The environmental services formula has increased from $200 per opening and $200 per closing of a case to $250 per open and $250 per close.
Uses and Restrictions
Funds from this grant will be disbursed up to the maximum allocation for Lead Poisoning Prevention and Response activities within each grantee’s jurisdiction. Use of funds is unrestricted.
Reports
Fee for Service: Quarterly electronic invoices must be submitted to the IDPH Lead Program, specifying lead case management home visits and environmental investigation openings or closures conducted during the reporting period. IDPH may conduct record reviews at any time during the grant period. Records must be kept in accordance with requirements set forth in the Lead Poisoning Prevention Code.
