Short Description
Provide outreach and care coordination services to follow up with individuals who have overdosed from opioids and connect them to appropriate treatment and recovery supports in the catchment area.

Objective
1. Meeting with regional community stakeholders to continue mapping local resources. 2. Begin meeting with treatment and recovery resources in target area to establish formal treatment referral relationships. 3. Grantee will provide a standardized case management reporting form to IDPH monthly. 4. Referrals will receive services.
