Objective
The Routine HIV Screening Development Grant is to build statewide capacity for new healthcare providers (HCP) including major medical systems, federally qualified health centers, local health department clinics, community-based organizations, college or university student health centers, and other healthcare provider networks to implement routine HIV Screening services in targeted communities currently lacking HIV routine screening services for underserved populations. Funds will support capacity building and training to assure new site establishment and service implementation. In addition, funds will support the delivery of a Department-approved, evaluated, cultural competency training course to reduce stigma against high incidence risk groups. This funding opportunity meets the current goals of the National HIV/AIDS Strategy (2022-2025), and the United States Preventative Services Task Force (USPSTF) to (1) increase the percentage of people living with HIV who know of their infection and (2) increase the percentage of newly HIV-diagnosed people promptly linked to care, partner services and prevention interventions. The CDC has established the importance of routine HIV screening in “Revised Recommendations of HIV Testing for Adult, Adolescents, and Pregnant Women in Healthcare Settings” guidelines. The USPSTF rated as Grade “A” the recommendation that all persons 13 to 64 years of age be annually screened for HIV regardless of risk at no cost to the persons for this service. This funding opportunity will support these goals by engaging more health care providers and building their capacity to offer culturally competent HIV screening services as part of general medical care.

This grant cycle will be competitively awarded for SFY2024 (Year 1) with the option of non-competitive renewal applications for SFY2025 and SFY2026. The grant term will be from July 1, 2023 through June 30, 2026. The Year 2 and Year 3 awards will be contingent on satisfactory applicant performance evaluations conducted by the Department each year of the grant term. Awards will also be contingent on adequate appropriation and project allocation for each year. The Project Outcomes listed below will be completed each year of the grant period. Outcome 1: Expansion of Routine HIV Screening Complete capacity building processes resulting in implementation and documentation of routine HIV screening by 6/30/2024 in 6 healthcare sites initially lacking HIV routine screening located in targeted Illinois communities. Two of these sites shall be in Suburban Cook. Two of the sites will be shall be within the Collar Counties. And two of these sites shall be in Illinois outside of the Chicago Eligible Metropolitan Area. Outcome 2: Enhance HIV Post-Diagnosis and Partner Services Ensure that within a minimum of 6 agencies already performing routine HIV screening, screening-identified HIV-diagnosed persons are promptly informed of their HIV-positive diagnosis, engaged into HIV medical care, offered Partner Services and referred to HIV risk reduction interventions and any needed social services. Outcome 3: Implement a Cultural Competency Program Address HIV stigma, homophobia and transphobia through providing evaluated curriculum-based training in 6 agencies providing routine HIV screening by 6/30/2024. These agencies must be healthcare providers with limited or no prior history of implementing an on-site HIV cultural competency training curriculum focused on these topics. Evaluate this training at each site to demonstrate increased staff self-efficacy in providing routine HIV screening with cultural competency in the workplace. At one selected site, evaluate the effect of the training on patient perceptions of cultural competency with respect to HIV status, sexual orientation and gender identity and on HIV-positive patient care engagement and retention rates. Outcome 4: Develop Routine HIV Screening Models and Materials Develop a training resource describing different models that agencies use to implement routine HIV screening in varying site types by 6/30/2024. The toolbox should be comprised of various training materials including but not limited to best practices for routine HIV screening implementation models in various clinic flows. Media for these materials may include webinars, printed materials, posters, and/or PowerPoint presentations. This collection should be made available through the grantee’s website and submitted to the Department by 6/30/2024 formatted for posting on the IDPH website.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active DUNS number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
Each application shall be objectively scored by a team of three reviewers, first individually, and then collectively for team consensus scores. Each team will include IDPH staff members with at least one from outside the IDPH HIV Section. The application shall be scored on the follow criteria with the following point weightings: Organizational Capacity and Experience (7 points), Project Narrative (28 points), Scope of work (20 points), Health Equity (35 points), Staffing and a Detailed Budget and Justification (8 points), Risk Assessment (2). An applicant score of 80 points or higher shall be considered eligible for funding.
Award Procedures
Upon successful application review, EGrAMS shall generate a Notice of State Award (NOSA) to the finalists who have successfully completed all grant award requirements and have been selected to receive grant funding. The NOSA will specify the funding terms and specific conditions resulting from applicable pre-award risk assessments. Successful applicants will receive an email notification from eGrAMS and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature in eGrAMS and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH. A Notice of Denial shall be sent to the applicants not receiving awards via eGrAMS. Upon final approval by IDPH of the eGrAMS grant application, the applicant shall be sent one grant agreement through eGrAMS. Applicant shall be required to print the grant agreement, sign it, and return it to IDPH (by mail or email) for execution. After execution, a copy of the executed grant agreement, along with a Grant Instructions and reporting requirements shall be returned to the applicant via email.

Uses and Restrictions
Out of state travel cost estimates must be approved in writing by the Grant Manager at least 1 week in advance of the travel as reasonable, appropriate and grant relevant. Equipment costs must be approved in writing by the Grant Manager at least 1 week in advance of the purchase. Funding may NOT be used for Syringes for Syringe Service Programs. Additional Grant Guidance: Funds allocated for subgrantee costs in this agreement may not be reallocated for lead agency expenses. The funds from this Grant shall be used only for providing support to the IDPH Routine HIV Screening Development Grant and should be restricted to the approved budget in the grant application. Changes and amendments to the use of funds must be approved by the IDPH HIV Section.

The Grantee shall ensure all testing data and positive case report data is entered and maintained into the Provide® Enterprise database. The Grantee shall be responsible for collecting and uploading site testing data from the sites into the Provide® Enterprise database on a monthly basis, using the Department’s instructions and specified format. The Grantee shall collect negative aggregate testing files from testing sites, clean data files, and conduct the upload of files into Provide® Enterprise on a monthly basis within 20 days of the close of the previous month. The Grantee shall be responsible for ensuring that the routine testing providers collect and enter all required positive test report variables into Provide® Enterprise within 7 days of the first reactive result. The Grantee shall be designated as an HIV Counseling and Partner Services agency authorizing their oversight of the conduct HIV positive follow-up and their access confidential name reporting for any HIV- positive clients identified through grant HIV screening. . The Grantee shall be required to take all IDPH trainings including Data and Security Guidelines training and other IDPH required training to be authorized to view names of those persons testing positive through routine testing on an ongoing basis.
