• Attendance at the technical assistance webinar is required to apply • Grant applications are submitted through IDPH’s Electronic Grants Administration and Management System (EGrAMS) • Award notices will be made to the agency contact in EGrAMS. The contact will receive an email notification of the availability of the grant agreement for download and signatures in EGrAMS • Objectives shall be specific, measurable, attainable/achievable, relevant and time bound, and activities supporting attainment of objectives shall be described. The program plan shall demonstrate innovative or new efforts. This award is for one year; this is the initial/competitive year for this program. This grant opportunity may be renewed up to 2 additional years conditional on need, satisfactory performance, and funding availability.

Criteria Selecting Proposals
Grantee must demonstrate expertise in and capacity for performing infection control assessments in health care facility settings, including refining/developing assessment tools as needed. Grantee must be able to analyze assessment and infection data and develop sound recommendations based on the evidence base, best practice, and state and federal regulations/guidance. Grantee must demonstrate ability to engage with and provide technical assistance to health care facilities related to improving infection prevention and control practices, including effective follow up with leadership and key facility staff and gathering or developing resources to share with facilities. Grantee must be able to evaluate effectiveness of the work performed. Grantee must have ability to travel within Illinois and must work closely with IDPH and CDC.

Project funds may be used for costs associated with planning, organizing, and the implementation of other program elements to build infection prevention and control capacity in Illinois. Recipients may only expend funds for reasonable program purposes, including personnel, travel, supplies, and services (e.g., contractual support). All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose.
Reports
• This is a CDC funded grant. The grantee will submit progress reports in accordance with federal reporting requirements and per updated guidance as directed by the CDC and the IDPH Division of Patient Safety and Quality. • Expense reporting and reimbursement certification forms should be submitted monthly or quarterly, as required. • An annual Federal Financial Report is required. • Final performance and Financial Status reports are due after the project period by CDC.

Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
