Short Description
Comprehensive Health Protection Grant will be a one-year combined grant agreement consisting of the Local Health Protection Grant (LHPG) and up to ten additional Office of Health Protection (OHPt) and Office of Disease Control grant programs.

Objective
Local health departments may apply for LHPG and additional grant programs (if qualified and funding is available) within the same EGrAMS application, and in turn the Department will execute one grant agreement.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
This is a non-competitive grant. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any required information will be returned to the applicant for correction and re-submission. Because the LHPG is a non-competitive, formula-based grant program, all eligible applicants will receive a grant award.
Award Procedures
Upon final approval by IDPH of the EGrAMS grant application, the applicant will be sent a grant agreement through EGrAMS. Each applicant will be required to print the grant agreement, sign it and return it to IDPH for execution. After execution, a copy of the executed grant agreement will be provided to the applicant.
Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

Uses and Restrictions
LHPG funds may be used by certified local health departments for any health protection program or service including, but not limited to, Infectious Diseases, Food Protection, Potable Water Supply, and Private Sewage Disposal per Section 615.210 of the Local Health Protection Grant Code (77 Ill. Adm. Code 615). LHPG funds are intended to supplement other Federal, State, and local funds available to support local health protection programs, including the four programs that must be assured. Provided the four programs are assured, the local health departments may use the LHPG funds for any health protection program, activity or service, or for shared management or administrative support costs (which must be detailed by budget line item). Grant funds for the additional SFY 2020-21 grant programs may be used by certified local health departments towards applicable programs and services intended in each grant program’s funding and eligibility requirements and program standards found in each program’s grant code or administrative rules.

Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
