Short Description
The Illinois Department of Public Health’s Division of Maternal, Child and Family Health Services (MCFHS) receives federal Title V funding each year to improve maternal, child, and infant health outcomes across the state. Funded activities align with the state’s action plan and selected priority needs. The Increasing Well-Woman Visits – Community (IWWV-C) Grant Programs were designed to address Illinois Title V priority of assuring accessibility, availability and quality of preventive and primary care for all women, particularly for women of reproductive age. More specifically, the goal of the grant is to increase the percent of women ages 18-44 with a preventive medical visit (well-woman visits) in the past year. A well-woman visit is an annual physical which includes preventive services, screening, counseling/discussion and immunizations. Well-woman visits should be comprehensive in nature and are important for all women across the life span. These visits play a critical role in screening for mental health conditions and in the management of chronic diseases. For women of reproductive age, these visits should also include discussion of a reproductive life plan and identification of activities to achieve optimal health prior to pregnancy if applicable. A variety of factors impact a woman’s ability to receive care including cost, availability of providers, transportation, competing priorities and health literacy (understanding the importance of preventive care and knowing how to navigate the health care system).

Objective
• Assist women 18-44 with accessing quality, comprehensive preventative care (e.g., Mammogram, pap and pelvic exams, emotional well-being, tobacco and substance use, violence and injury prevention, sexual health and healthy relationships, and physical health and health promotion). • Support evidence-informed guidance, to address behavioral, social, and environmental determinants of health. • Assist communities with assessing the barriers to women scheduling preventative care visits. • Increase awareness of the importance of well-woman visits for at least 75% of staff at grantee organization.

Application Procedures

Criteria Selecting Proposals
Good standing status with the Illinois Department of Public Health, Office of Women’s Health and Family Services shall be assessed upon receipt of application. Good standing must be maintained throughout the grant cycle. Proposals will be reviewed by program staff. Grants will be reviewed by program staff during a competitive review process. The categories are as follows: • Scope of Work: Community need; Applicant capacity; Target audience clearly defined; Plan provided with detailed activities • Work Plan: Activities, outcomes and measurements are provided and aligned with program goals and objectives; Information provided in the SMARTIE format • Budget::Budget reasonable and justified.
Award Procedures
These grant applications are considered competitive and will be scored. Those receiving low scores may not receive funding or may receive an amount less than requested.

3 - 6 months
Appeals
For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer.
Renewals
There are no renewal awards for this grant program. Grantees must submit a new application for each grant cycle.
Uses and Restrictions
No indirect costs allowed. Only 10% of the grant may be allocated to Direct Administrative Costs. State of Illinois Grant Awards will not allow reimbursement of pre-award costs, or construction costs
Reports
Monthly Reimbursement Forms will be due on the 15th of the following month; Quarterly Progress Reports will be due on the 30th of the month following the closure of the quarter; and End of Grant Report will be due forty-five days from the last date of the grant period. All reports and monthly reimbursement forms shall be submitted through the online grants management system.

Audit requirements per JCAR Title 44 Illinois Administrative Code7000.90 or alternative audit requirements specific to program.
Records
Grantees are required to maintain grant accounting records 3 years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later

Program Accomplishments
