Short Description
The Fetal Infant Mortality Review (FIMR) initiative is a nationwide systems strategy supported by the American College of Obstetricians and Gynecologists (ACOG) that aims to improve services, systems and resources for women, infants and families across the state of Illinois. The program seeks to accomplish this goal by convening a multidisciplinary review team and a Community Action Team (CAT) of diverse community leaders. The review committee will systematically review fetal and neonatal deaths within targeted communities of the state. Reviews will include identifying non-medical factors that contribute to fetal and neonatal loss and subsequent adverse pregnancy outcomes and developing recommendations. The CAT will design and implement key interventions based on the recommendations generated by the review committee. Reviews, recommendations and subsequent interventions will place an emphasis on eliminating disparities in perinatal, infant, and maternal health; directing resources; and proposing interventions that improve access to, utilization of, and full participation in comprehensive perinatal and women's health services, particularly for women at higher risk for poor health outcomes. Cases for review will be referred through the statewide Adverse Pregnancy Outcomes Reporting System (APORS) and other sources such as provider agencies and birthing hospitals in Illinois.

Objective
Program Objectives: • Assure that the FIMR program is appropriately established and able to obtain, protect and store the confidential medical information of its participants; this includes annual IRB approval by institution and revising/updating consent forms as needed. • Maintain and enhance a data-informed FIMR program, which includes standardized processes for case identification, case selection, outreach activities, interviews and medical record abstraction; ensure that the FIMR program will include fetal deaths of at least 20 weeks gestation and infant deaths up to one year of life. • Program should identify at least one opportunity to align with national FIMR recommendations and propose strategies to be implemented. • Follow, and revise as needed, a program logic model, and implement an evaluation plan. • Increase community awareness of, and engagement with, the FIMR program by developing, updating and appropriately branding materials and reports that describe the FIMR program. • Establish an action plan and convene diverse stakeholders to formalize a community action team to implement the recommendations.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Criteria Selecting Proposals
Good standing status with the Illinois Department of Public Health, Office of Women’s Health and Family Services shall be assessed upon receipt of application. Good standing must be maintained throughout the grant cycle. Proposals will be reviewed by program staff
Award Procedures
Upon completion of the submission and review process, each successful applicant will receive a grant agreement to be signed by the entity’s authorized official. The grant agreement is not binding on the parties until it has been fully executed by the Illinois Department of Public Health. Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

Formula Matching Requirements
Matching funds (cash/in-kind contributions) are not required by the applicant. If applicant receives cash/in-kind contributions, these need to be included in the total operating budget reported in the online grant management system and specified in the grant budget summary
Uses and Restrictions
No indirect costs allowed. Only 10% of the grant may be allocated to Direct Administrative Costs. State of Illinois Grant Awards will not allow reimbursement of pre-award costs, or construction costs All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program unless specifically allowed under grant program guidelines. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for repayment of those funds used for the prohibited purpose. Regardless of the source of funding (federal pass-through or State), all grant-funded expenses must be compliant with Cost Principles under Subpart E of 2CFR200 unless an exception is noted in federal or State statutes or regulations. Allowability Allowable – All grant funds must be used for items that are necessary and reasonable for the proper and efficient performance of the grant and may only be used for the purposes stated in the grant agreement, work plan, and budget. Items must comply with all applicable state and federal regulations. Allocable – Grant-funded costs must be chargeable or assignable to the grant in accordance with relative benefits received. The allocation methodology should be documented and should be consistent across funding sources for similar costs. Reasonable – The amounts charged for any item must be reasonable. That means the nature and amount of the expense does not exceed what a prudent person under the same circumstances would expend; and that the items are generally recognized as ordinary and necessary for the performance of the grant.
Reports
Monthly Reimbursement Forms will be due on the 15th of the following month; Quarterly Progress Reports will be due on the 30th of the month following the closure of the quarter; and End of Grant Report will be due forty-five days from the last date of the grant period. All reports and monthly reimbursement forms shall be submitted through the online grants management system.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
