Short Description
The Illinois Department of Public Health is taking steps to address a key strategy of the State of Illinois Opioid Action Plan which calls for, strengthening data collection, analysis and sharing to better identify opportunities for intervention. The purpose of this grant is to inform drug overdose prevention efforts in Illinois by enhancing statewide surveillance of opioid overdose deaths through the State Unintentional Drug Overdose Reporting System (SUDORS). The goal of this program is to enhance understanding of opioid overdose deaths, including underlying factors, and inform prevention activities at the local, state, and national levels. The grantee will abstract retrospective data from death certificate and coroner/medical examiner reports. This effort will facilitate longitudinal trending of opioid overdose death data and enhance ability to identify relevant factors associated with increases. The grantee will disseminate SUDORS data and expand the Block-by-Block program
This is a non-competitive grant funded through a grant from the Centers of Disease Control and Prevention (CDC) “Overdose Data to Action-States” (OD2A-S) CDC-RFA-CE-23-0002 and is contingent upon federal funding. The grantee will work in collaboration with the IDPH Office of Health Promotion (OHPm), Division of Emerging Health Issues’ Violence and Injury Prevention Section to ensure alignment with state and federal goals and objectives.

30 ILCS 708, TITLE 44: CHAPTER I: Section 7000
Objective
These funds support jurisdictions in collecting high quality, comprehensive, and timely data on nonfatal and fatal overdoses and in using those data to inform prevention and response effort. Funding is essential to increase capacity for investigation and reporting on unintentional opioid overdose death cases as well as gaining an understanding of the role synthetic opioids in opioid overdoses. Services provided by the grantee are critical to fulfilling the statewide objectives set forth in the Illinois Overdose Action Plan. The anticipated outcomes are: 1. Collect data on all Illinois unintentional drug overdose death from death certificates (DC) and coroner/medical examiner (CME) and toxicology (TOX) reports and enter case-level SUDORS data into the CDC’s web-based data entry system accessed through CDC’s Secure Access Management Services (SAMS) 2. Report data to CDC following semi-annual established reporting periods. 3. Collaborate with the CDC to improve SUDORS data quality. 4. Disseminate SUDORS data for prevention. 5. Distribute funds to Coroners/Medical Examiners to support enhanced toxicological testing. 6. Expand the Block by Block (BXB) initiative.

Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification are required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Criteria Selecting Proposals
State Unintentional Drug Overdose Reporting Project Year 24, Grant are non-competitive grants. All eligible applicants will receive a grant award. There are no merit-based review criteria for evaluating the applications. Each application will be reviewed for completeness and accuracy. Applications lacking any information required under Section D (2) above will be returned to the applicant for correction and re-submission. The grantee must work closely with CDC and IDPH Office of Health Promotion (OHPm), Division of Emerging Health Issues’ Violence and Injury Prevention Section leadership, attest to meet grant deliverables, provide adequate budget and justification for allocation of project resources.
Applications will be reviewed for content, work plan activities, budget proposals, and required application supplemental material. In addition, based on adherence to prior grant guidelines, timely submission of reports and fiscal monitoring. The Health Equity Checklist is a scored portion of the IDPH grant application. In public health, health equity is the opportunity for everyone to reach their full health potential, regardless of any social determined circumstance. There are seven questions worth 35 points.

Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for recouping of those funds used for the prohibited purpose. Expenditure reports must be submitted quarterly. To be reimbursable under the Department/Office of Health Promotion Grant Agreement, expenditures must meet the general criteria outlined in the NCFI
Reports
Grantees are required to submit quarterly performance reports on their work plan objectives and quarterly fiscal reporting in EGrAMS within 30 days after the reporting period. Reimbursement reports cannot be approved unless progress reports are up to date. Failure to submit required reports in a timely manner will result in delays with approval of reimbursements. The grantee will ensure quarterly reports are submitted
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
The Grantee must work closely with IDPH Office of Health Promotion (OHPm), Division of Emerging Health Issues’ Violence and Injury Prevention Section and the CDC and participate in calls and meetings as scheduled. Quarterly performance reports will be required to summarize activities performed, findings and common themes from health needs assessment surveys, steps taken to address gaps, results of technical assistance, and priorities for next steps. Grantees will also submit quarterly expenditure and reimbursement reports. Grantees are required to submit quarterly performance reports on their work plan objectives and quarterly fiscal reporting in EGrAMS within 30 days after the reporting period. Failure to submit required reports in a timely manner will result in delays with approval of reimbursements. The grantee will ensure quarterly reports are submitted.
