Short Description
Tooth decay and gum disease are common yet preventable oral health problems. The purpose of the Oral Health Promotion Program (OHPP) grant is to develop and implement innovative programs that address the oral health needs of children and families through high-quality prevention services and information. This program opportunity is primarily intended to reach low-income vulnerable families with a focus on early opportunities that reduce tooth decay and other oral health conditions and help people get oral health care services when needed. This grant provides support to Illinois communities by granting funds, providing technical assistance, and training. Through this funding opportunity, local health departments and other public not-for-profit service providers will develop and implement community-based oral health programs or expand current community-based oral health programs. The program is designed to assist school personnel, individuals and families by assuring access to oral health education, fluoride varnish, All Kids enrollment, and referral to a dental home.

Objective
The program objectives are tied to Healthy People 2030 and Oral Health Section (OHS) objectives: to increase the number of children and families who are educated on self-care, obtaining access to professional care; to increase the proportion of children, pregnant moms, and families whose oral health is stable and/or improved; to increase the number of low-income children who have a preventive dental visit (HP2030: OH-09); to increase the proportion of children and adolescents who have dental sealants on 1 or more molars (HP2030: OH-10); to increase the appropriate use of the oral health care system (HP2030: OH-08 and AHS-05); to reduce the proportion of children and adolescents with lifetime tooth decay (HP2030: OH-01); to reduce the proportion of children and adolescents with active and untreated tooth decay (HP2030: OH-02); to reduce the proportion of adults with active or untreated tooth decay (HP2030: OH-03); to reduce the proportion of people who can’t get the dental care they need when they need it (HP2030-AHS-05); to reduce the consumption of added sugars by people aged 2 years and over (HP2030: NWS-10); to increase the proportion of children in kindergarten, second, sixth, and ninth grades receiving required school dental examinations (OHS objective); to increase the proportion of eligible children benefitting from All Kids oral health benefits (OHS objective); and to increase the number of children and women during pregnancy who are connected to a dental home to receive appropriate oral health care (OHS objective).

Applicant Eligibility
Applicants must be governmental agencies or tax-exempt organizations. Eligible applicants are those health departments, school districts, and community-based organizations that received Dental Sealant Grant Program funding in prior years or are identified as serving a priority population or special area of need in the state of Illinois.

Application guidance for this program is approved by the Office of Management and Budget every three years. Applicants review the current Guidance for requirements in applying annually to receive Title V Maternal and Child Health Block Grant funding.
Preapplication Coordination
Pre-application coordination between a State Maternal and Child Health Agency and other State and local entities within the State is at the discretion of the State. States are required to have mechanisms in place for obtaining public input and feedback on the development of their Title V Maternal and Child Health Block Grant applications and Title V supported programs. All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
The pre-application phase of this grant will ensure that applicants have the experience, skill, and capacity to execute grant deliverables and outcomes.
Award Procedures
The grant application will be reviewed and evaluated by Illinois Department of Public Health, Division of Oral Health Staff and funded based on project need as demonstrated in the application. Award notices will be made to the agency contact listed in EGrAMS. The agency contact will receive an email notifying them that the grant agreement is available for download and signature through the EGrAMS system. Payments will be made based reimbursement certification forms submitted quarterly. Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health

Uses and Restrictions
This grant provides support to Illinois communities to develop programming for populations who are most at risk for dental diseases by granting funds, providing technical assistance, and training. Through this funding opportunity, local health departments and other public not-for-profit service providers will develop and implement community-based oral health programs or expand current community-based oral health programs. The program is designed to assist school and WIC personnel and families by assuring access to oral health education, fluoride varnish, All Kids enrollment, and referral to a dental home. The program also helps families comply with the mandatory school dental examinations for children in kindergarten, second, sixth and nineth grades. All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program. The use of grant funds for prohibited purposes may result in loss or recovery of grant funds.
Reports
The grantee will be required to submit quarterly performance progress reports and quarterly reimbursement certifications within 30 days after the reporting period. Quarterly reports are to include a short narrative of project progress and a data report that consists of the following: 1. Measurable data (i.e., number of people reached and how reached); 2. Basic demographics about the population served (number of children, number of pregnant persons, number of adults); 3. Breakdown by geographic area (county as well as urban vs rural); 4. Any improved outcomes and how you measured them. (Baseline data will be needed to report this) The final report/reimbursement request must be received by the Illinois Department of Public Health no later than July 31, 2025. Failure to submit required performance reports may cause a delay or suspension of funding.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).

Program Accomplishments
During the first funding cycle of fiscal year 2024 (from October 1, 2023, to December 31, 2023), 12,411 pregnant, postpartum, school-aged children and families received health promotion and oral health education instruction, along with necessary materials. Our grantees are targeting the MCH population and provide community events and in-person presentations. Over 7,000 social media postings regarding self-care were viewed, while case management and sharing of oral health resources were made a priority, along with school collaborations about oral health. Dental homes were set up for WIC, pregnant, postpartum, children, and families, along with partnerships to increase access to care. Over 11,000 children and pregnant persons received direct dental services. Workforce training was prioritized and included fluoride varnish application, continuing education courses, and the expansion of the Public Health Dental Hygienist model. The grantees distributed oral health kits with instructional information to WIC clients, children, and adolescents. The focus was placed on the medically compromised physically or mentally on Medicaid and those who are homeless or refugees.
