Short Description
The Mobile Dental Program uses a repurposed, fully equipped dental health van to provide on-site comprehensive dental services to vulnerable and underserved pediatric and adolescent populations on the campuses of Advocate Children’s Hospital in Oak Lawn and Advocate Children’s Hospital in Park Ridge. The van serves as a modern dental office on wheels, equipped to provide virtually any service provided in a freestanding dental facility.
The Dental Health Van enables the mobile dental staff to maintain appointment schedules at both children’s hospital campuses. Most of the pediatric and adolescent patients seeking care are covered by Medicaid or have no insurance and other challenges limiting access to quality oral health care such as providers, prevention education, routine dental care (exams, x-rays, cleanings/fillings) and treatment.

Objective
To provide mobile dental services that achieve measurable improvements in the oral health outcomes of underserved populations.

Applicant Eligibility
To be eligible, applicants must be registered in the State of Illinois GATA system and pass the grantee pre-qualification, fiscal and administrative risk assessment (required once annually), and the programmatic risk assessment, and meet the criteria: the only eligible applicant for this grant is Advocate Illinois Masonic Medical Center dba Advocate North Side Health Network.

Reports
Quarterly performance and financial reports, due within 30 days of the end of each quarter.
Records
Grantee shall maintain for three (3) years from the date of submission of the final
expenditure report, adequate books, all financial records and, supporting documents, statistical
records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.334, unless
a different retention period is specified in 2 CFR 200.334 or 44 Ill. Admin. Code §§ 7000.430(a) and (b). If any litigation, claim or audit is started before the expiration of the retention period, the records
must be retained until all litigation, claims or audit exceptions involving the records have been
resolved and final action taken.
