Short Description
Two years into the Covid-19 pandemic, testing remains critical to our pathway out of this crisis and return to normal. Rapid antigen tests are an effective public health tool to help us quickly identify positives and isolate and support those who need it. Increasingly, the constraints that have prevented their widespread use—including supply and price—are being addressed and overcome. One of the biggest challenges we face is and will be access: ensuring our most vulnerable and hard-to-reach citizens can access free tests. The price of a test or where someone lives should not prohibit access to an essential tool in our fight against a virus that continues to take the lives of nearly 2,000 Americans each day. To address this constraint, IDPH will issue a grant to The Rockefeller Foundation (RF) to implement Project Access Covid-19 tests (Project ACT) through its public charity, RF Catalytic Capital (RFCC). The goal of Project ACT is to help IL provide free Covid-19 rapid tests to households in vulnerable and hard-to-reach communities.

Objective
IDPH will provide a grant for supply of delivered tests, indicating the zip codes (based on Social Vulnerability Index (SVI) rankings) where residents will be able to requests tests.

Application Procedures
All applications must be submitted through EGrAMS (idphgrants.com). All sections of the application must be completed, and the application must be validated, certified, and submitted through EGrAMS. If the Budget includes cash or in-kind amounts, the source of those funds must be identified. Grantees must also submit or upload other required documents and attachments, including but not limited to, organization W-9 form and Sub-Contractor/Sub-Grantee Authorization form(s). Grantees shall receive an executed grant agreement for the grant program applied for in the EGrAMS applications.
Criteria Selecting Proposals
The PROJECT Access to COVID 19 Tests (ACTs) -Phase II Testing 2023 grant is non-competitive. The named eligible applicant will receive an award. There are no merit-based review criteria for evaluating the application. The application will be reviewed for completeness and accuracy.
Award Procedures
A Notice of State Award (NOSA) shall be issued to the finalists who have successfully completed all grant award requirements and have been selected to receive grant funding. The NOSA will specify the funding terms and specific conditions resulting from applicable pre-award risk assessments. The Illinois Department of Public Health (IDPH) is exempt from utilizing the standard NOSA issued on the GATA Grantee Portal. Successful applicants will receive an email notification from EGrAMS and must review the funding terms and specific conditions in the grant agreement and accept utilizing an electronically signature. Both the electronic signature in EGrAMS and a physical signature on the grant agreement must be completed by an authorized representative of the grantee organization and submitted to IDPH. A Notice of Denial shall be sent to the applicants not receiving awards via EGrAMS.

Grant funds shall be used for approved costs as demonstrated in the most recently approved grant application budget in EGRAMS.
Reports
The grantee shall provide periodic financial and periodic performance reports in EGRAMS as directed.

Audit requirements will be provided under the terms of the Grant Agreement to be issued.
Records
Pursuant to the terms of Grant Agreement, the Grantee must retain grant records and make those records available to the Department upon request.
