PROGRAM INFORMATION
Short Description
The FY25 Preventive Health and Human Services Block Grant, funded by the U.S. Centers for Disease Control and Prevention includes sexual offense set-aside allocations are issued per grant period by federal partners to states and jurisdictions. This grant award will provide the Illinois Coalition Against Sexual Assault (ICASA) with resources to address Healthy People 2030 (https://odphp.health.gov). The current HP2030 goals we are addressing are Goal: Prevent violence related injuries and deaths: • Reduce adolescent sexual violence by anyone — IVP 17. Current national status: getting worse. • Reduce sexual or physical adolescent dating violence — IVP 18. Current national status: getting worse. • Reduce nonfatal child abuse and neglect — IVP 16. Target met or exceeded. These objectives will be met by providing administrative assistance to rape crisis centers to conduct sexual violence prevention programs; ensure the centers are compliant with education and fiscal requirements; and ensure quality of statewide prevention education through training and technical assistance with local rape crisis centers. Also, the amount should be 267,477 in all places it looks like it auto populated from a previous form in one section. I am working on getting the appropriation code and will send it when I find out the information.

Grantees approved policies must meet the federal PHHS requirements of 45 CFR Part 75, as applicable. Preapplication Coordination
All grantees are required to register with the State of Illinois through the Grant Accountability and Transparency Act (GATA) website, www.grants.illinois.gov, complete a prequalification process, and be determined "qualified" as described in Section 7000.70. Registration and prequalification is required before an organization can apply for an award. The entity is "qualified" to be an awardee if it: 1) has an active UEI number; 2) has an active SAM.gov account; 3) has an acceptable fiscal condition; 4) is in good standing with the Illinois Secretary of State, if the Illinois Secretary of State requires the entity's organization type to be registered. Governmental entities, school districts and select religious organizations are not required to be registered with the Illinois Secretary of State. Criteria Selecting Proposals
Adequate work plan and budget to meet the overall goals of the program. Non-competitive application will be sent for revision if criteria have not been met.The application will be reviewed by the Department grant monitor. Upon final application approval, a grant agreement will be sent to the applicant for partial execution. When returned to the Department, the agreement will be processed and fully executed. A final copy will be forwarded to the applicant.
Award Procedures
Grant will be awarded upon acceptance of the Notice of State award by signing the Uniform Grant Agreement by the grantee and the Director of the Illinois Department of Public Health. Grantee will receive an executed grant agreement within 30 days of approved application

Appeals
Merit-Based Review Appeal Process For competitive grants, only the evaluation process is subject to appeal. Evaluation scores or funding determinations/outcomes may not be contested and will not be considered by the Department's Appeals Review Officer. To submit an appeal, the appealing party must: o Submit the appeal in writing and in accordance with the grant application document through IDPH's Merit-Based Review Appeal Request Form available in the following link https://app.smartsheet.com/b/form/ed4d113385de41feb38964a8005ce72b. Appeals must be received within 14 calendar days after the date that the grant award notice was published. Appeals must include the following information: The name and address of the appealing party, Identification of the grant, A statement of reasons for the appeal, If applicable, documents or exhibits to support statement of reason The IDPH Appeals Review Officer (ARO) will consider the grant-related appeals and make a recommendation to the appropriate Deputy Director as expeditiously as possible after receiving all relevant, requested information. The ARO must review the submitted Appeal Request Form for completeness and acknowledge receipt of the appeal within 14 calendar days from the date the appeal was received. o The ARO will utilize an Appeal Review Tool to consider the integrity of the competitive grant process and the impact of the recommendation. The appealing party must supply any additional information requested by the agency within the time period set in the request. The ARO shall respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
Uses and Restrictions
All grant funds must be used for the sole purposes set forth in the grant proposal and application and must be used in compliance with all applicable laws. Grant funds may not be used as matching funds for any other grant program unless specifically allowed under grant program guidelines. Use of grant funds for prohibited purposes may result in loss of grant award and/or place the grantee at risk for repayment of those funds used for the prohibited purpose. Regardless of the source of funding (federal pass-through or State), all grant-funded expenses must be compliant with Cost Principles under Subpart E of 2CFR200 unless an exception is noted in federal or State statutes or regulations.
Reports
Monthly fiscal and quarterly program reporting, as specified in the grant agreement is required.
Grantee will submit monthly expenditure reports within 30 days of the end of each month.
Audits
Article XV Audit Requirements: Audits Grantee shall be subject to the audit requirements contained in the Single Audit Act Amendments of 1996 (31 USC 7501-7507) and Subpart F of 2 CFR Part 200, and the audit rules and policies set forth by the Governor’s Office of Management and Budget. See 30 ILCS 708/65(c); 44 Ill. Admin. Code 7000.90.
Records
Grantee shall maintain for three (3) years from the date of submission of the final expenditure report, adequate books, all financial records and, supporting documents, statistical records, and all other records pertinent to this Award, adequate to comply with 2 CFR 200.333, unless a different retention period is specified in 2 CFR 200.333 or 44 Ill. Admin. Code §§ 7000.430(a) and (b).
